CAT ADOPTION APPLICATION

fax 401-247=7570

Adopting a cat is a lifetime commitment and responsibility. Our goal is to match our available cats and kittens with loving individuals and families that can provide a quality, permanent home and that are well suited to meeting the animal’s individual needs. We are happy that you are interested in giving a home to one or more of our kittens/cats. In order to ensure that you and your new cat will be well suited to each other please read and answer each question carefully. Please be advised that we are a private, all volunteer rescue organization and as such reserve the right to deny your application if for any reason we feel this would be in the best interest of the animal. In order to complete an application you must be at least 18 years of age, be able to demonstrate that you are able to have a cat where you live and understand that we will only consider an application from the person with whom the cat will live. Our animals are adopted out “fully vetted”. This means that they have received a thorough veterinary exam, all age appropriate vaccinations, been treated for parasites, spayed or neutered, and been tested for feline leukemia and FIV in addition to having had any and all other needed emergency and routine care – all veterinary records will be provided. The value of this veterinary care alone is well in excess of $400.00. Given this we do have a minimum adoption donation of $75.00 for adults and $100.00 for kittens which helps defray some of our costs, this will allow us to assist additional stray animals in our area. Please let us know if this fee would be a hardship and we will work with you on an agreeable arrangement. To begin the adoption process, answer the following questions completely. Then you may submit your application via e-mail or fax. Please note that information provided will be verified.

I have read and understand the questions on the application and recognize that any false, deceptive or missing information may be cause to void the application. I also understand that my application may be denied if in the opinion of the adoption volunteer it would not be in the best interest of the cat. I give my permission for a volunteer to verify all information on the application.I understand that the process of verifying my information will include contacting my veterinarian for a reference as well as speaking with my landlord if applicable.

I understand that owning an animal is a long-term commitment that requires mature and careful consideration. I assert that I have the financial means to care for this animal, including providing any and all needed medical care. I also understand that the animal I am applying for may have been abandoned, surrendered, born outside or removed from a shelter and therefore, there may be limited information regarding the animal’s background, including their medical and behavioral history.

I realize that the cat will take some time to get used to me and to my household and I will not hesitate to ask for advice from my veterinarian or a shelter volunteer. HOWEVER, SOMETIMES AN ADOPTION IS NOT A GOOD MATCH AND IF I DETERMINE THIS TO BE SO, I AGREE TO RETURN THE CAT ONLY TO THE BARRINGTON PARTNERSHIP FOR ANIMAL WELFARE AT ANY TIME.

I agree to provide the animal with all food, shelter and veterinary care as required by Rhode Island and/or Massachusetts state law. I understand that to finalize the adoption process I will be required to provide positive identification and sign the application in front of a volunteer. By signing below, or submitting this form via e-mail I certify the truth and completeness of all information I have supplied.
