Dear Parent/Guardian:
Your child may be eligible for free tutoring services through the No Child Left Behind supplemental educational services (SES) program. Eligibility is based on free-reduced lunch status and enrollment in the following schools: Greene-Hills, O’Connell, South Side, and Memorial Boulevard. This notice is being distributed to help you understand SES and what you need to do to take advantage of these services for your child.
SES TUTORING IS FREE AND YOU GET TO CHOOSE
One of the most important features of SES is that parents are the ones who will make choices for their children. You’ll be able to choose from different types of services given by different providers at different locations. That way, you can decide what’s best for your child. There is no obligation to enroll your child in SES.
LEARNING MORE ABOUT SES PROVIDERS
Attached to this letter is a list of state-approved providers that will work with Bristol students. You may wish to review their programs at the websites provided or contact them directly at the telephone numbers listed before making your choice. You may also want to review the document offering tips on how to select a provider.
QUESTIONS YOU MAY WANT TO ASK PROVIDERS
On the last page of this packet, we have included information on how to choose a provider. You may also want to ask the following questions:
· When and where will the tutoring take place?
· How often and for how many hours in total will my child be tutored?
· Do you provide transportation? Bristol Schools do NOT provide transportation.
· Do you provide snacks after school?
APPLICATION DEADLINE
If you decide you want to enroll your child in SES, complete the green “Application for SES Tutoring Services” and mail or hand deliver the form NO LATER THAN JANUARY 20th to your child’s school or to me at Bristol Board of Education, 129 Church Street, PO Box 450, Bristol, CT 06010, ATTN: Kristin Irvine. If we do not receive the form postmarked on or before January 20, 2012, your child will be placed on a waiting list in case of any openings during the year.
SES AVAILABILITY
There are only a limited number of spaces in the SES program, so we may not be able to enroll every child whose parent applies.
PRIORITY FOR LOWEST ACHIEVING STUDENTS
In compliance with federal law, we will give first priority to lowest achieving students. The achievement level will be based on the student’s score on CMT. We may also look at additional assessment data to make our decision.
NOTIFICATION DATE
Confirmation or denial letters will be mailed on the week of January 23, 2012.
NEXT STEPS
Academic records and contact information will be given to your selected provider. They will
contact you to set up a meeting with you and your child to review his/her needs and develop an individualized learning plan. If you do not hear from the provider within two weeks of submitting your application to our office, please contact me at 860-584-7071 so that we may follow up.

HOW DO I CHOOSE
A SUPPLEMENTAL EDUCATIONAL SERVICES (SES) PROVIDER FOR MY CHILD?
FIRST:
· CAREFULLY READ information in the provider directory. Call, email, or check the websites of any providers that interest you.
· NOTE the number of tutoring hours available for your child in the last column. Each child has a $1495 maximum allotment for tutoring services. Depending on the hourly rate, your child may get more or less tutoring hours for the year.
NEXT, CROSS OUT the names of the providers that:
· DO NOT provide tutoring at the grade your child is in;
· DO NOT tutor in the subject area(s) you choose for your child. If you are not sure which subject(s) to choose, look at your child’s assessment records or talk to the classroom teacher;
· DO NOT tutor when you want tutoring;
· DO NOT offer the location that you want for the services;
· DO NOT provide transportation if you need it.
THEN:
· Attend the SES EXPO if you are available. Talk to the providers and learn about their services.
· IDENTIFY FIRST AND SECOND CHOICES. In most cases, you will get your first choice, but there are some instances where we must go to a second choice. If you do not list a second choice, your child may not receive services.
· READ and COMPLETE the Supplemental Services Application form.
· RETURN the completed pink application to your child’s school or to the Board of Education, 129 Church Street, PO Box 450, by December 23, 2011.
Important:
1) You must sign the GREEN application and deliver directly to your child’s school or Board of Education by January 20, 2012 in order for your child to receive SES. Without your signature, we cannot forward your information to the selected provider. No one but the provider, your child’s school, and Central Office will receive any information about you and your child.
2) Please be sure your address and contact information is correct so we can reach you if necessary.
