Please be aware that even if a single document is missing, incomplete, unreadable, inconsistent or not original, your request will not be processed and you will need to ask for another appointment.

The Italian Embassy is not authorized to obtain certificates from U.S. Vital Recordsor vital records certificates of your ancestors, you should contact the Italian Municipality (Comune).

LONG FORM

U.S.- issued vital records (birth, marriage, and death certificates) must be submitted in "long form" or “extended form” which can be obtained from the Vital Statistics Office of the State in which the birth/marriage/death occurred.

APOSTILLE

Each document must also have affixed an APOSTILLE (According to Hague Convention of de. 5th, 1961). Legalization provided by the Office of the Secretary of State of the Country where the certificate is issued. Please note: it is not a stamp on the certificate.It is a legalization that is a separate form, stapled to the birth/marriage/death/ certificate.The Apostille does not need the translation.

TRANSLATION

all certificates must be submitted with the official in the Italian language (except for the apostille- look on references on the right hand side of the screen). Conformity of the translations to the originals must be certified by the Italian Consulate competent for the State that issued the document.

N.B.

The U.S. Certificates of Naturalization and the letter of “no naturalization records” issued by the U.S. Citizenship and Immigration Services (USCIS) do not need apostille or translation.

NO US DOCUMENTS

All documents from COUNTRIES OTHER THAN THE U.S.A. must comply with the local regulations on the legalization of documents, they must be translated into Italian and the translation certified by the Italian Consulate/Embassy in the Country where the document was issued (may contact any Italian Embassy/Consulate in the world by logging on to the pertinent web sites. DOCUMENTS ALREADY PROVIDED

If you are applying to us after a relative of yours has already been granted Italian citizenship by descent from the same ancestor as the one from which you are claiming citizenship, you must still submit all the family documents required, or authenticatedcopies issued by the Consulate or Municipalities (Comuni) that were used in your relative’s application for Italian.

INCORRECT INFORMATION and DISCREPANCIES

It is your responsibility to ensure that all documents contain the correct information. Incorrect certificates may delay the processing of your application or affect your eligibility for Italian citizenship

Double-check all documents word by word to make sure that there are no discrepancies or mismatches in names, last names, dates and places of birth and ages. If there are major discrepancies in such fields, you must have those certificates amended through an official "affidavit to amend a record”, to be obtained from the Vital Records Office that issued the document or through a competent Court Judge.

N.B.

Some U.S. vital records offices may require applicants to provide a letter from the Consulate stating the reason the ancestral certificates are being requested. In this case, we suggest that you present the U.S. office a copy of this information kit.
