Notice of a transfer review
Part 5, Article 18 of the Children & Families Act 2014 (Transitional and Saving Provisions) (No.2) Order 2014
This letter is to notify you that the County Council is undertaking an EHC assessment of (NAME DATE OF BIRTH / / )
The annual review meeting scheduled to take place on / / (date) at
School/College /Setting will be a transfer meeting.
As part of the assessment process the local authority is proposing to use existing assessment information and reports where they remain sufficient.
Please confirm in writing to the Setting, School, or College
· that you will attend the review to provide an update in person, or
· that you are satisfied that your previous report remains sufficient, or
· that you will be submitting an updated report (to be submitted to the education provider 2 weeks prior to the transfer meeting) .
Your response and any additional updates or reports will be submitted to the LA as part of the transfer meeting documentation.
When submitting updated reports please can you use the template enclosed –thank you.
