Please complete the application form in legible block capitals and black ink or typescript. The layout should not be changed or altered in any way.

6. CONDUCT AND CONFLICTS OF INTEREST
Before you complete this section, it is important that you read paragraphs 10 – 12 and the leaflet from the Commissioner for Public Appointments Northern Ireland in the accompanying application pack.
