This is an invitation to reflect, as honestly as you can (or wish), about your ministry as a Reader. It is intended to be a gift, so that, in conversation with your reviewer, you can reflect on that ministry creatively and constructively. The conversation is confidential to you and to your consultant. You are invited to send the summary form to your Assistant Warden (or where there is no-one in post to the Warden). I do hope that you will find this helpful. It is not intended to be a burden: more a chance to reflect confidentially with a colleague and to highlight what you need.
