3. Does the event have temporary electrical structures, e.g., are there large generators or alterations to standard electrical supplies? / Yes ☐ No ☐
4. Does the event have any marquees/structures greater than 9m2 (e.g. spectator stands, stages, ramps, TV screens, cubby houses, climbing walls, etc.? / Yes ☐ No ☐
5. Does the event have temporary staging over 1m high or lighting and support systems? / Yes ☐ No ☐
6. Is the event longer than 8 hours? / Yes ☐ No ☐
7. Will there be any noise sources (i.e., band, PA’s) that are likely to impact surrounding residents or businesses? / Yes ☐ No ☐
8. Does the event have any pyrotechnics? / Yes ☐ No ☐
9. Will adequate lighting be provided at the event where required? / Yes ☐ No ☐
10. Is there any aspect that may provide a specific concern from a public health perspective, e.g., high risk displays, moshing, road closures, crowd movements, strenuous exercise, likely drug use, motorbike displays, tattoo demonstrations, traffic congestion, likely impacts heat/wind or commercial amusement rides? / Yes ☐ No ☐
