Reports to: / Managers – Programs and Information Services
Principal Function(s): /
  • Develop, implement and evaluate strategies to build referrals to the 1800 ASTHMA Helpline service according to performance indicators and timeline
  • Contribute to the implementation of the 1800 ASTHMA Helpline service, to benefit people with asthma and their carers across Queensland and New South Wales.
  • Contribute to the implementation of the Foundation’s Integrated Care programs.

Role Area / Responsibilities/behaviours / Performance Outcomes Sought
Develop, implement and evaluate strategies to build referrals to the 1800 ASTHMA Helpline service /
  • Ensure delivery of project commitments according to timeline
  • Develop and maintain relationships with key stakeholders, including but not limited to PHNs, hospitals, community health and general practice
  • Develop and implement strategies to drive referral of patients with asthma from health servicesto 1800 ASTHMA Helpline
  • Deliver evidence based information and support services through 1800 ASTHMA Helpline (see below)
  • Coordinate the project plan, evaluation strategies, data analysis and report writing on outcomes

  • AFQN delivers on project targets and commitments, including supply of reports according to agreement

Quality information services /
  • Deliver evidence based information and support services through 1800 ASTHMA Helpline, including:
  • Quality implementation of procedures and processes to ensure provision of evidence based information to people with asthma and their carers
  • Quality implementation of procedures and processes to ensure quality and consistent data collection and entry
  • Contribute to the quality implementation of the COACH program via 1800 ASTHMA Helpline
  • Contribute to the development and evaluation of resources to support consumers and health professionals in asthma management

  • AFQN delivers a range of information services that are evidence based, responsive to patient need and meet data collection and reporting requirements including Helpline services and education workshops
  • AFQN produces a range of quality, evidence based materials for consumers, health professionals and the community that are based on need
  • Patients experience improved asthma control after their Helpline encounter

Plan, deliver and evaluate AFQN integrated care programs /
  • Develop, promote and implement a systematic program of integrated health care initiatives across the primary and tertiary sector to enhance asthma related health
  • Deliver education and training sessions to health professionals across the state

  • Timely delivery of planned outcomes to meet key performance indicators

Relationship development and maintenance /
  • Support the development and maintenance of relationships with the primary and tertiary healthcare sector to ensure appropriate consultation towards AFQN’s integrated care portfolio
  • Contribute to identification of gaps in care integration and seek to embed AFQN services and programs at primary and tertiary health care level

  • Productive relationships with key stakeholders within primary and tertiary healthcare sector
  • Integration activities are developed in response to identified need and in collaboration with stakeholders

  • Assist the Managers – Programs and Information Servicesas and when required with ad hoc projects

Skills, Qualifications and Attributes required / Essential
  • Formal qualifications in health
  • Current health professional registration
  • Project co-ordination experience including strategy development, implementation, evaluation and report writing
  • Demonstrated experience in the delivery of information sessions and workshops
  • Strong organisational skills and the ability to manage competing priorities
  • Competency in Microsoft Office suite
  • Australian Driver’s License
Desirable
  • Health promotion experience or qualification
  • Recent experience in clinical delivery of respiratory care
  • Experience with telephone based patient support programs
  • Certificate IV in Training and Assessment (or willingness to rapidly acquire)
