  1. Acopyofeachslotmachine’srespectiveparsheet,indicatingtheprobabilityofhittingeachawardandtheassociatedpayforeachawardavailable,mustbemaintainedinsideofeachslotmachine.
    WhilecheckingforPARsheetsitwasnotedthatmachine1006didnothaveacopyofthegame’sPARsheetinsidethegame.

ICO RECOMMENDATION:

TheslotdepartmentwasimmediatelynotifiedandadvisedtogetareplacementPARsheetforthegame.EachmachineonthefloorshouldbecheckedtoensurethattheproperPARsheetismaintainedinthemachine.

MANAGEMENT RESPONSE:

TheslotdepartmenthasbeeninstructedtoverifythatPARsheetsareineachslotmachineandthatthePARsheetcorrespondstothecurrentgame.TheslotdepartmentistoprovideawrittenreporttotheICOnolaterthanMM/DD/YYwiththeresultsoftheircheckandattestationthat the correct PARisbeingmaintainedineachmachine.

NONCOMPLIANCEISSUE:

  1. Aprogressivejackpotlogmustbemaintainedforallprogressivebanks. Thislogreflectsthelicenseename, applicablemonthandyear,andidentificationofthebank. Thelogmustbeusedtorecord,onadailybasis,theamountoftheprogressiveliability(theadvertisedjackpot,andanysecondaryjackpotprogressiveliabilities),aswellasanyprimaryandsecondaryhandpayjackpots. Statewideprogressivesdonotneedtobelogged.

OriginallogforMonth,Yearhadbeendroppedintheaccountingboxinsteadofthecopy. AccountingpersonneldidnottransferdatafromMonth1sttothe4thandgettheoriginalbacktothefloor. ProgressivedataislostfortheperiodofMonth1–4,Year.

ICORECOMMENDATION:

Accountingpersonnelneedtohavefurthertrainingonthenecessityofreviewingprogressivelogsandensuringthatcorrectionsarenotedontheoriginallogswhenneededinadditiontothecopiesrevieweddaily.

Thisprovision,inwholeorinpart,maybewaivedbytheDirector.” Thetop lightonslotmachine11619didnotilluminatewhenthemaindoorwasopened.

ICORECOMMENDATION:

TheSlotDepartmentManagerneedstoensurestaffischeckingslotmachinesforproperoperationofrequiredlights.

MANAGEMENTRESPONSE:

SlotDepartmentManageriscurrentlysettingupandschedulingfloorpersonpreventivemaintenancealongwiththeslottechniciansdoingtheirpreventivemaintenanceonallgamesthroughoutthecasino. Inadditiontothisslotmanagementwillbewalkingthefloorandaddressingthesetypesofmaintenanceissues. Theseissueswillalsobeloggedontherepairlogthattheslottechnicianswillberesponsibleforcheckingonadailybasis.
