School Registration Checklist
New Students
Proof of Age Requirement
o Copy of Child’s Birth Certificate
Proof of Residence
o Gas, electric, or water bill from last 60 days that includes name and address
o Mortgage or lease statements
o Valid driver’s license or state-issued ID showing current address
Proof of immunizations and Health Records
o Immunization Records
o Physicals and Medical Needs
o Evidence of needed medication or restrictions
Caretaker Proof of Identity
o A copy of enrolling adult’s identification information
o Legal documents outlining custodial or guardianship arrangements
Prior school documents
o Report Cards, transcripts, or assessment information
o Gifted documentation
o Special Education documentation, IEP, or 504 plans
On-line Registration
o Create parent account in PowerSchool
o Complete on-line registration forms
o Complete waivers for book rental and meal
o Make payments that are applicable
o Pick up textbooks (high school only)
Parental Involvement
o Attend Open House and Orientations
o Schedule appointment with guidance counselor or principal
o Attend parent-teacher conferences
o Use PowerSchool on a regular basis
