EMPLOYEE CONFLICT OF INTEREST STATEMENT

The undersigned employee of the School Board of Walton County, Florida, herebystates andagrees

as follows:

(1)If he or she is acting in the capacity as a purchasing agent for the School

Board of Walton County, Florida, he or she has not and will not directly or indirectly purchase,

rent or lease any realty, goods or services for the said School Board from any business entity of

which he or she or his or her spouse or child is an officer, partner, director, or proprietor or in

which he or she or his or her spouse or child has a material interest.

(2)He or she acting in a private capacity has not and will not rent, lease or

sell any realty, goods, or services to the School Board of Walton County, Florida.

(3)He or she or his or her spouse or minor child have not and will not, at

any time, accept any compensation, payment or thing of value when he or she knows, or with the exercise of reasonable care, should know, that it was given to influence a vote or other action in which he or she was expected to participate as an employee of the School Board of Walton County, Florida.

(4)He or she, as an employee of the School Board of Walton County, Florida, has

not and will not have or hold any employment or contractual relationship with any business

entity or agency which is subject to the regulation of, or is doing business with said School

Board, excluding participation by the undersigned in negotiations of a collective bargaining

agreement with the School Board. He or she has not and will not have or hold any employment

or contractual relationship that will create a continuing or frequently recurring conflict between

his or her private interests and the performance of his or her duties as an employee of the School Board.

(5)In the event that a conflict of interest occurs, the employee will immediately notify theschool principal or department supervisor by written notification. The school principal or department supervisor will notify either the Director of Human Resources or the Chief Financial Officer by written notification.

The undersigned employee of the School Board of Walton County, Florida hereby executes this statement indicating that it is true and correct and that they have received a written copy of this document. If the employee is aware of any potential conflict of interest please identify below.
