  • Capacity building grant has gone to the Public Health branch – no decision as yet
  • Accessing Vic health data was challenging due to privacy restrictions – Cheryl to follow up to facilitate future access
  • Agreement for standing item on “Child and Youth Participation” to inform HCP. Emma H to take lead assisted by Jane C. Due to current workloads a specific working group was postponed but may evolve at a later stage as a result of standing item.
  • New planning cycle for 2012 – 2015 may incorporate early years and youth services
  • Kate Somerville and Bernadette McGrady were not in attendance to present a case study on young people with disabilities, so thisitem has been postponed. As Kate was mistakenly left off the minutes of the last meeting and Bernadette is currently unable to attend HPC meetings, some miscommunications occurred here.
  • Terms of Reference have undergone HPC membership review. Small grammatical changes were noted and working group format changes will be noted
  • Agreement for standing item reports to be emailed to Troy prior to meetings to allow for more discussion time
  • Terms of Reference were unanimously accepted and ratified by attending HPC members

  • Family Violence prevention was presented to the Frankston North Community Renewal – as a result the project will also present at the community safety forum
  • This was also presented to Hastings Community Renewal which was also well received.
  • Rob Koch (Better Men Australia) has been consulting for Dads in Distress. “Date with Dad” is scheduled to run in Frankston North, Hastings and in conjunction with Frankston City Council. St Anne Parish School – Seaford is working directly with Dads in Distress to present the “Date with Dad”.
  • White Ribbon Day working group is up and running. Five agencies and two community groups are reviewing promotion through:
-Billboards
-Team sponsorship – Arthurs Seat Challenge – Youth Team
-Rotary
-Rosebud buses
  • Elder Abuse project –New Victorian guidelines have been released for responding to suspected Elder Abuse
  • Project will be participating in the Age and Well Expo
  • Social Inclusion Working Group has now divided into two new groups:
(a) Community Transport Network & (b) ABC – Act Belong Commit Working Group
  • Mike reported on Transport network – Multiple expressions of interest have been received for the volunteer transport collaborative. Dept of Transport and Dept of Human Services have also expressed interest in a collaborative transport model for MorningtonPeninsula and Frankston to follow.
  • An initiative grant has been submitted for $75K, which may allocated to fund a 0.8 EFT Community Transport Worker for 18 months
  • Troy reported on ABC – The SDC held an extraordinary meeting to sign the MOU for ABC between Curtin University and the FMPPCP.
  • Working group members have been emailed the forthcoming agenda, Terms of Reference, budget and project timeline.
  • A “train the trainer” model is being utilised for the ABC program.
  • It was suggested that membership to this working group be opened to the wider collaborative.
  • The HPC members to consider an appropriate consumer (community) representative to attend HPC meetings. Stronger Communities may provide a community rep.
  • Needs assessment is in its final stages of completion – this should be ready to launch at the next HPC
  • SDC are undertaking a new planning cycle for 2012 – 2015
  • Member agency forum was held with a focus on the potential for “One Catchment, One Plan”
  • From the forum a sub-committee has been formed as a catchment planning group
  • Peninsula Medicare Local – announcement on next successful LGA’s is due in approx 2 – 3 weeks. A Transition Board has been working on this project and has representatives from: GP Network, Peninsula Health and thePCP(Terry PalioportasPeter Brookehouse)
  • Nikki presented – Priority areas have been agreed upon:
-COPD
-Depression, anxiety
-Diabetes
  • Obesity was also noted and will be a priority for phase two
  • Steering committee is currently being established. Stakeholder forum proposed for 2012 with DeakinUniversity who are the collaborating centre. Postcode obesity will be evaluated along with showcasing good practice.

  1. PCP Member Agency Forum
/ Debrief, outcomes, planning process discussion /
  • In principle commitment to “one catchment health plan”, although this presents challenging complexities with multi-levels of key stakeholders.
  • Helen Kelleher will conduct the data collection tasks and a forum is proposed for March 2012.
  • Agencies may contribute to the data analysis by providing Helen with published research that will inform the findings. Further information will be available in due course.
  • The final report is scheduled to be submitted to DHS – approx June 2012.
  • Juliette Fritzl has extracted some key themes from the forum to be established:
-Vision, expectations and scope
-Expected benefits for key stakeholders
-Governance framework
-Support for stakeholders to inform the process
-Background paper to synthesise frameworks
-Build evidence base to inform process
-Community engagement strategy – with particular focus on smaller agencies and consumers (community members)

  • Separate reporting periods are forthcoming for the HPCPCP
  • Reporting against plan – previous 12 month review
  • Documents are available as templates – for impact measures/capacity building measures
  • This is due to be ratified by the SDC within the next fortnight
  • Last years PCP case study on Family Violence was acknowledged and well received
  • Capacity building meetings are forthcoming

  • Brotherhood of St Laurence will be holding a forum on social inclusion/social support – targeted at Aged Care. Mike Hilier to provide flyer for circulation.
  • Connect You – project exhibition begins on 30th November for agency invites, Emma H to provide save the date details. Schools will attend on the following day with the exhibition to remain open at Frankston Arts Centre – Cube 37 until 20th December 2011
  • Communities for Children are undergoing a review of its Community Strategic Plan for 2012 – 2014. In line with the new Family Support Guidelines, consultations will take place at Anglicare Frankston, Level 2, 60 – 64 Wells St, Frankston on:

  • Due to complications with inbox limits, it was suggested and agreed that the HPC would trial for a two month period the uploading of health promotion program and events informationto the FMPPCP webpage for members to keep up to date with health promotion news.
  • HPC to review at next meeting whether direction to web links is working as an effective information sharing method.
