Applications must be typed using 12-point font, and should be free of spelling and
grammatical errors. The MDC mentorship program requires a 3.0 G.P.A., though
applicants with G.P.A.’s between 2.75 and 2.99 will be considered if there are extenuating
circumstances. Thus, an applicant with a G.P.A. between 2.75 and 2.99 must complete
Essay 4 and explain any circumstances that adversely affected academic performance.
The MDC Mentorship Committee reserves the right to hold interviews with applicants
prior to admittance into the program.

Essay 1: Please write briefly (150 words or less) about how the mentoring program would
benefit you. Please, also explain how you consider yourself as coming from an underrepresented
group.
Essay 2: Please write briefly (150 words or less) about your career goals.
Essay 3: Please write briefly (150 words or less) about why you chose to become a psychology
major.
Essay 4: Note. Only complete this section if your GPA is between 2.75 and 2.99. In this
statement (150 words of less), explain any circumstances that have negatively impacted your
GPA. Also, include any actions you have taken to improve your GPA.
