PROJECT WORKER
JOB DESCRIPTION
Role: Project Worker – Chalkhill Road and New North Road
Responsible to:Registered Care Home Manager and Deputy Project Manager
Location:Wembley and Islington
Key Responsibilities and Tasks Include:
Residents
- Participate in the referral processin partnership with referring agencies: by interviewing, assessing, and compiling accurate reports on prospective clients.
- Take the lead role in providing keywork services to an allocated number of residents and work with them on a variety of issues as set out in the keywork guidelines and the individual careplan.
- Complete needs and risk assessments on individual residents. Develop comprehensive care plans and risk management plans
- Work closely with andprovide leadership to, Support Workers. This includes delegating relevant keywork tasks to them as agreed with the Project Manager. Provide full information to Support Workers when planned leave is taken in order that they may take on keywork duties entailing more responsibility with the allocated residents.
- Review and update care and risk plans specific to individual residents’ needs in consultation with the residents and other partner agencies.
- Ensure that all key clients have an opportunity to set-out their own agenda for managing their mental ill health, via the Recovery Approach and set-out in the relevant planning format.
- Liaise with other agencies,participate in meetings to present reports as required in order to help residents fulfil their needs as set out in the care plan.
- Ensure allocated residents are in receipt of all benefits to which they are entitled.
- Keep other relevant members of staff informed on issues concerning allocated residents.
- Administer residents’ oral medication and record and monitor residents’ medication when required.
- Operate the sanctions, evictions and appeals procedure concerning residents and ensure all breaches of the resident's licence agreement are reported to the Project or Deputy Project Manager and take such immediate action as is necessary to deal with a problem.
- Keep abreast of issues concerning all residents in the Project.
- Ensure the ongoing welfare of all residents and report any concerns or complaints which might suggest a vulnerable adult is at risk of harassment or abuse, using the St Martin’s Safeguarding Procedures.
- Ensure all paperwork concerning your allocated residents is up to date
- Co-ordinate regular room checks, Health & Safety Inspections and fire drills, reporting any maintenance issues as and when appropriate, using the relevant systems.
ADMINISTRATION/FINANCE
- Undertake other administrative tasks as allocated by the project management.
- Keep account of the individual petty cash float and to submit claims when appropriate.
Team and organisational involvement
- Attend staff meetings on a regular basis.
- Ensure that duty cover is provided by participating in the rota system and provide emergency cover at the Project as necessary.
- Attend and participate in other meetings within the organisation as appropriate and as agreed by the line manager.
- Represent the Organisation and attend meetings with outside agencies as agreed by the line manager.
- Work with all service users, staff and outside agencies in accordance with the organisation's Equal Opportunities policy.
- Attend supervision and participate in the annual appraisal meetings with the line manager
- Attend training as agreed by the line manager, ensuring you are up to date with the required core training
- Participate in the recruitment, induction and training of new staff members as appropriate.
- Take on such other duties as are consistent with the post as required by the line manager.
- Adhere to all the organisation's policies and procedures.
- Follow the organisation's Health and Safety and Maintenance procedures.

HolidaysYou will receive 25 days leave each year, in addition to statutory public holidays for the first two years of service; you will then be entitled to 28 days; holiday. from your fourth year you will be entitled to 30 days.
Probationary Period6 Months
Hours37.5 per week on average, including unpaid 30 minute break per shift. Start and finish times will be in accordance with the weekly rota, which will include weekend shifts and bank holiday shifts.
PensionSt Martin of Tours operates a contributory Group Personal Pension Scheme. At the end of the probationary period, St Martins will double the employee’s contribution into the group scheme up to a total of 6% of gross annual salary.
