In Virginia, these include the Chronic Disease Self-Management Program (CDSMP), the Diabetes Self-Management Program (DSMP), Tomando Control de su Salud (Spanish CDSMP), the Cancer: Thriving and Surviving Program (CTSP)and the Chronic Pain Self-Management Program (CPSMP).
