2015 Scholarship Information

Available Scholarships:

·  $1000 – 12th grade student entering an accredited college or university (vocational or academic)

·  $ 250 – 8th grade student entering an accredited Catholic High School

·  $ 50 – 8th grade student of a Catholic Home School Program entering 9th grade coursework

Requirements:

·  Registered parishioner of St. Anthony Catholic Church for a minimum of 1 year

·  Maintained a 3.0 GPA or grade average of 80 or higher (transcript required):

o  College applicants – 9th through 11th grade and 1st semester of 12th grade

o  Catholic High School applicants – 7th grade and 1st semester of 8th grade

o  Catholic Home School applicant – 7th grade and 1st semester of 8th grade

·  Complete the Jean Davidson Memorial Scholarship Application

·  Complete Transcript Request Authorization & submit to school

o  CDA must receive transcripts by the application deadline of April 22, 2015

·  Essay - Neatly written or typed, not to exceed 500 words. Must not mention your name or make other identifying reference within or on the essay.

o  2015 Topic - “How do you, as a teen, embrace the sacrifice of living the Catholic Faith among peers”

Additional Information:

·  Scholarships will be awarded in May.

·  If no qualified candidate, no scholarship shall be awarded that year.

·  With proof of registration to a school of choice the scholarships will be paid directly to the school no later than September 15, 2015.

Awards will be based on the following evaluation:

·  Attendance and Participation in Religious Education / 20 pts.
·  Participation in Parish Ministries and Activities / 20 pts.
·  Community Service / 20 pts.
·  Essay - / 20 pts.
·  Scholastic Grades (transcript required) / 20 pts.
