1. Summary of post
This post is one of three SpR posts in Paediatric Hepatology. Appointment is for a fixed period only until February 2009.
2. Birmingham
Birmingham is a progressive, modern city offering pleasant accommodation and excellent civic infrastructure. There are good road, rail and air links and easy access to the countryside. The International Convention Centre, Symphony Hall, National Indoor Arena and ArtGallery and several Theatres are centrally located. There are excellent schools and the Universities of Birmingham, Aston and Central England offer a wide range of educational facilities and other activities.
3.The Birmingham Children's Hospital NHS Foundation Trust.
The Birmingham Children's Hospital NHS Foundation Trust is the Regional Paediatric Centre for over 5 million people in the West Midlands, Britain's largest health region. It has approximately 300 beds and cots, including 20 paediatric intensive care beds, and is one of the largest children's hospitals in the country. The Trust provides secondary level paediatric services for the child and adolescent population of much of the city of Birmingham, both in the hospital and through its community and clinical support services. It is also the regional referral centre for the West Midlands region and many of its specialist services extend their referral base beyond the regional boundaries. It has a national and international reputation for high grade paediatric care going back to the early years of this century and many important innovations in modem paediatrics originated here. Approximately 9,000 inpatients and 50,000 outpatients are treated at the hospital each year. The hospital moved in May 1998 to its present, central city site. In addition to upgraded facilities, the site includes a parent accommodation block (Edward House), the JamesBrindleyHospitalSchool and a new Education Centre. Recent developments included opening of a dedicated Burns Unit and a purpose build extension of the Education Centre.
4.The University of Birmingham
Academically, the University is organised into seven faculties, one of which is the Faculty of Medicine and Dentistry. This faculty is headed by the Dean and is administratively split into the Dental and MedicalSchools. The MedicalSchool comprises 6 divisions encompassing 26 departments, which cover the whole range of pre-clinical and clinical subjects. Interdisciplinary research is encouraged, as are links between the clinical and basic science departments in other faculties. The MedicalSchool is justly proud of its library and reference facilities and has good working relationships with Trusts and Health Authorities.
5.The Academic Department of Paediatrics/ The Institute of Child Health
The University Academic Department of Paediatrics and Child Health (Head of Department - Professor Tauny Southwood) is part of the Division of Growth and Development. The Academic Department and is situated in the Clinical Research Block, Whittall Street, adjacent to the Children’s Hospital in the centre of Birmingham.
Education and Research Directorate
A new Directorate of Education and Research has recently been established in order to consolidate and support education and research initiatives within the Trust. The Directorate includes the Professional Development Team, Training and Organisational Development, Medical Education and Research and Development. The Directorate is led by the Medical Director for Education and Research (Professor Deirdre Kelly).
Research and Development
The Research and Development Department manages research in the Trust and is responsible for research governance, including research ethics issues. This includes managing research finances, ensuring drug trials are carried out to international standards and co-ordinating laboratory involvement in research. The Department also co-ordinates an R&D education programme within the Trust and provides guidance for developing and registering research projects, research design, statistical advice and data analysis, including laboratory co-ordination of samples and pharmacy support for clinical trials.
6. The Liver Unit
The Paediatric Liver Unit at BCH comprises a multi-disciplinary team, which provides a supra-regional service for children with liver disease, acute liver failure and liver and small bowel transplantation.
The Unit is based on Ward 8 and has 13 beds including a 3 bedded high dependency unit, 6 isolation cubicles, one dedicated elective surgical bed, one dedicated small bowel transplant assessment bed and two rapid through-put low dependency beds for elective admissions.
The unit works closely with both the transplant service at the University Hospital of Birmingham NHS Foundation Trust, and the Department of Paediatric and Child Health, section of gastroenterology and nutrition, at the children’s Hospital.
The philosophy of the Liver Unit is to provide family centred care for children and their families. There is a strong multi-disciplinary Team to support this philosophy.
The Liver Unit cares for over 300 children with viral hepatitis and there is a strong multi-disciplinary team led by Mrs Jaswant Sira, Hepatitis Liaison Nurse and Mrs Maxine Brown, Hepatitis Liaison Nurse

8.FACILITIES:
MEDICAL.
The Liver Unit is based on Ward 8 and has 14 beds which includes a 3 bedded high dependency unit and 6 isolation cubicles, three rapid through-put low dependency beds for elective admissions and one bed for elective small bowel transplant assessment.
There is a purpose built endoscopy suite in the main theatre block which is furnished with modem video endoscopy equipment. There are thrice weekly sessions under general anaesthesia. In addition there are facilities for ERCP.
Intensive care is based in the modem, multi-disciplinary Intensive Care Unit which includes isolation facilities for post transplantation management.
Close links are maintained with adult hepatology services with joint clinics established for inherited metabolic disease and for hand-over of adolescent/adult patients.
SURGICAL.
There is a designated transplant theatre with all support facilities for transplantation and hepatobiliary surgery.
There are particularly close links with the Liver Transplantation Unit at the UniversityHospital, Birmingham. The units have shared infra-structure for transplant co-ordination and organ retrieval. There is shared consultant cover for transplant surgery with the team from the UniversityHospital who contributes to pre and post operative care of transplant patients.
9.DUTIES OF THE POST
The successful candidate will be expected to work closely with the transplant and hepatobiliary surgical teams.
Applicants will possess MRCP Part 2 or MRCPCH Part 2 or equivalent, and will be expected to have experience in paediatrics at Specialist registrar level, Some experience of paediatric gastroenterology would be desirable.
Duties will include the management of children with all forms of liver disease, the peri-operative care of children receiving liver transplantation, and liver and small bowel transplantation.
He/She will be expected to provide support for the nurse led telephone follow up clinic.
It is expected that the candidate will work with two other Specialist Registrars, a Senior Clinical Fellow, two Senior House Officers and one Trust Doctor SHO level. The hospital will move to a Hospital @ Night out-of-hours service system in August 2008 and, in terms of on-calls, the appointed candidate will contribute to the long day and weekend service only as shared jointly between hepatology and gastroenterology,at present 1:6, with prospective cover.
ON-CALL COMMITMENTS
The postholder will be expected to undertake on-call duties. The basic working week is 40 hours. The Birmingham Children’s Hospital NHS Foundation Trust has implemented a Hospital at Night System for out of hours cover. The hepatology SpRs participate in a rota providing middle level specialty cover to hepatology and gastroenterology patients between 17:00 and 22:00 Monday to Friday and 08:30 to 22:00 Saturday and Sunday. The postholder would participate in this rota. The hours of work will not exceed an average of 48 hours per week
Research and Academic Activities
It is expected that the post holder will take part in the academic activities and audits of the Unit. There will be opportunities for both clinical and scientific research.
The post holder will be expected to contribute to undergraduate medical student teaching and to play a role in postgraduate medical education and the training of other relevant staff groups.
The Department has close clinical and research interest with the University Hospital Birmingham NHS Trust. The successful candidate will be encouraged to take part in these activities as appropriate.
Research interests of the Liver Unit:
A commitment to research is encouraged within the Liver Unit. A comprehensive list of current research interests is available, these include:
- Therapy of viral hepatitis
- Quality of life following liver transplantation and small bowel transplantation
- New treatments for portal hypertension
- Pathophysiology and treatment of bone disease in chronic liver disease
- Non-invasive markers of rejection following small bowel transplantation
- The molecular basis of inherited metabolic liver and/or bowel disease
- Evaluation of alternative immunosuppression protocols and monitoring tools
- Evaluation of various aspects of Cystic Fibrosis associated liver disease
10.Equality and Dignity

11.Communication (Staff with supervisory responsibility)
An integral part of the role of any manager or person with leadership responsibilities is to communicate effectively with their staff and colleagues. It is an expectation of this role that resources and time will be allocated to communicate fully with staff and involve them in the decisions affecting them.
Arrangements should be made to ensure that local and Trust wide matters are communicated and discussed via appropriate means ie, team meetings, written briefings etc.
12. Induction
It is the responsibility of every employee to participate fully in induction.
A Trust wide induction course is held monthly and local induction will be provided within your own place of work.
A formal induction process will be arranged by your Clinical Lead/Programme Director within the Integrated Child Care Group and a consultant mentor will be appointed.
13.Appraisal and Performance Management (Medical Staff)
For Consultant Medical Staff an annual appraisal and review of the Job Plan is a contractual requirement.
All staff will be expected to fully participate in this process. This obligation will include the preparation for and attendance at appraisal interview and completion of associated documentation.
Failure to participate in any stage of the process will render the appraisal ‘incomplete’.
14.Health And Safety
Staff have a legal responsibility not to endanger themselves, their fellow employees or others by their individual acts or omissions. The post holder is required to comply with the requirements of any policy or procedure issued by the Trust in respect of minimising the risk of injury or disease.
15.Terms and Conditions of Service
The appointment will be held in accordance with the terms and conditions of service of hospital medical and dental staff and general Whitley Council terms and conditions of service. The successful candidate will be required to undergo relevant pre-employment checks including health screening, police check and Asylum and Immigration Act check. Basis salary will be within the NHS Career grade doctors’ scale dependent on previous experience. The post holder will be required to reside within ten miles of Birmingham Children’s Hospital unless otherwise agreed by the Chief Executive.
