2017 Annual Employee Awards
To be Presented October 18, 2017
TEAM AWARDS:
· Self Determination: This is the team that assists individuals to control their own lives, pursue their goals and dreams, and manage their resources. Extra credit for innovative ways to achieve these goals.
· Community, Mutuality & Service: This is the team that exhibits working together effectively as a team. They do their scheduling together which also involves covering for each other. They problem solve together, they plan events together, and there is a presence of strong leadership at the counselor level. Extra credit is given for demonstrating ways to build partnerships beyond Jubilee employees.
· Wellness Award: This team has demonstrated a commitment to their own nutrition, exercise, and overall well-being, as well as that of Jubilee individuals
· Tool of Success: This team is using the Jubilee tools to create success. They have set and accomplish goals related to the use of Therap, MITC, and other uses of technology to communicate as a team, and with and about individuals and with their families.
· Spiritual Care: This team has worked to address the spiritual lives of both the individuals and the staff. They make arrangements or accompanies individuals to Churches or Synagogues or to religious activities such as Faith & Light. They aid in helping the individuals to build partnerships with their place of worship and aid in their prayer life.
INDIVIDUAL AWARDS:
· Family Partnership Award: This staff member demonstrates exceptional skill communicating, partnering, and earning the confidence of individuals’ family members. Nominations from families are especially valuable for this award.
· Exceptional Performance: This staff member regularly goes above & beyond the call of duty. They demonstrates exceptional dedication, creativity, innovation, organizational achievement, teamwork, and builds partnerships.
· Respect & Human Dignity: This staff member provides exceptional service and makes a difference in the lives of persons served and their fellow employees. They treat people with respect, and show evidence of valuing people’s human dignity.
· Exceptional Customer Service
This person provides excellent customer service and treats all customers with a friendly and respectful attitude and tries to resolve their problem or question as efficiently as possible. He or she is responsive and communicative.This person goes above and beyond the call of duty to resolve outstanding issues that impact the overall agency and welcomes suggestions and ideas on how they can make Jubilee great. He or she not only responds to immediate issues or complaints, but continuously looks for ways (big or small) to improve processes to prevent future problems.
· Building Natural Supports in the Community
This person has demonstrated a commitment to facilitating and ensuring that our individuals develop natural supports (friends, co-workers, church community, volunteers, and neighbors) that enrich, broaden, and nurture their lives and will persist despite staff turnover. Provide stories and examples of these connections.
COMMUNITY MEMBER AWARD:
· Community Member Award: This person has made significant contributions to Jubilee individuals or Jubilee services. Some examples would be a medical doctor, an employer, a coordinator of community support (CCS), or a neighbor. This person would not be a Jubilee employee, family member of an individual, Board member, or a Jubilee volunteer.
· Volunteer Award: This person has made significant contributions to Jubilee individuals or Jubilee services. They could be an official volunteer in the Jubilee volunteer program or they could be in a less formal role.
OFFICIAL RULES:
1. Program Managers will be asked to nominate at least two employees, the Director of Program Services will nominate at least one employee, and all employees except the Executive Director are allowed to nominate or be nominated for any of the awards. Employees may not nominate themselves. Families are encouraged to nominate for the Family Partnership Award as well as any other award. The Client Council is encouraged to make nominations.
2. Employees can only win an award every other year.
3. Employees can only win the same award, in a specific category, every three years.
4. Employees must have at least one year of service with Jubilee.
5. There will be no house awards.
6. The Executive Director and the individual winners from 2016 will choose people from the nominations. They will consult with members of the management team as part of the decision making process.
7. Individual winners will receive a certificate and a net check for $600. All taxes and payroll withholdings will already have been deducted. Team prizes are $1,200 split amongst established team members. (The Community Member and Volunteer awards are not cash prizes.)
8. More than one person may win in any category but some awards may not have any winners. Tim Wiens, Executive Director, shall receive nominations no later than 5 p.m. on Wednesday October 4, 2017. Nominations shall be e-mailed to , using Microsoft Word.
9. Nominations must include the name of the person being nominated, a short description (less than one page) of detailing why the person should receive this award and the name of the person making the nomination.
10. The awards will be presented on Wednesday, October 18th at our agency-wide staff retreat.
11. 2016 individual winners were Audrey Lamp, JoAnn Erbie, Lamin Yilla, Priscilla Charles, Vicki Pendleton, Brandon Bell, Francisca Keyan, Rose Mudd, Julia McCune, Deb Garrison, Sakiru Salami, and Anwar Younas.
12. Examples of winning nominations from past years will be posted on the J: drive. Part of winning is telling a good story and giving evidence of why a team or individual is worthy of the award.
