Read the following brief scenarios, thendocument each interaction using your program’s progress note form, or the IFSP outcome form. Based on the scenario, you should determine which method of documentation is most appropriate for the situation.
After completion, your supervisor will review your documentation and discuss with you.
Scenario 1
Lance is a 29-month-old who has been receiving services at daycare for about a year. You see his mother, Erica, about once every two months at the daycare and you talk to her by phone every few weeks. Erica called you today to tell you that she will begin a new job in two weeks that requires some late hours. She needs to find a new daycare that can keep Lance until 7 pm. You give Erica some names and phone numbers of daycares that you heard have extended hours. You tell her that you will do some more investigating to see what else you can come up with and will call her in a couple of days if you find out about more daycares that would work for the family.
Scenario 2
Elia is a 15-month-old girl has been receiving services at home with her mother, Amy, for 9 months. She has a heart defect and was referred for some problems with feeding, swallowing and poor weight gain. Right now, Elia’s ECI services are speech therapy and nutrition. Amy is now concerned that Elia is not walking. She would like a physical therapist to see if Elia’s motor development is on track. You tell her that you will need to get a prescription from Elia’s doctor and will arrange a PT assessment as soon as possible. You will fax a prescription request to the doctor when you get back to the office and will schedule a PT eval. You will call Amy by next week with some potential dates.
Scenario 3
Christina is a 30 month old girl who has been receiving services at home for two months. Her mother, Sarah, called to say they received a letter stating their electricity was going to be cut off the next day. You tell Sarah that you will call her back with a list of agencies that may be able to help. After researching 211, you find three churches in Sarah’s neighborhood that provide utility assistance. You call Sarah back and provide her with the information. Sarah reports that she will contact the agenices. You call Sarah back later that day to see if she needs additional assisantce. Sarah reports that oen of the churches she contacted will pay her electricity bill.
