GENERAL POSITION DESCRIPTION:

The Social Services Coordinator is a full-time position that assists participants with entitlements, oversees resident funds, facilitates admissions into facilities, manages authorizations for service and advocates for participants when needed. The position will report directly to the CAO.

ESSENTIAL FUNCTIONS:

Fundamental Responsibilities:

·  Oversees all program participant funds; ensures spending in accordance with state and federal rules; audits resident funds on site on a quarterly basis; ensures Medicaid/Medicare/Social Security guidelines are adhered to as applicable; works with management, guardians and program participants in spend down situations.

·  Ensures participant eligibility for benefit programs as dictated by local, state and federal rules. Complies with wage reporting requirements to maintain eligibility.

·  Works with new admissions in residential program to ensure payee ship, resident funds, authorizations, HUD paperwork, benefits, etc. are in place to meet program needs.

·  Ensures authorizations for service delivery are current and in place so timely billing can occur for all programs. When and where needed, monitors authorization usage so as to not over deliver service.

·  Ensure HUD paperwork for HUD residential facilities is completed upon admission and annually thereafter for all participants.

·  Maintains and spot audits all resident fund paperwork at the office, including bank account information and usage as well as ledgers for each participant for cash on hand at the homes. Ensures each home complies with monthly turn in schedule.

·  Advocates for program participants and is a liaison to families, guardians and friends.

·  Is the liaison with regard to PCS assessment scheduling, appeals, authorizations and communication to participants and staff as needed.

·  Completes all required reporting to DSS and Social Security as required.

·  Performs back up billing functions as necessary.

·  Prioritizes and organizes responsibilities. Displays the ability to address several problems or tasks at once.

·  Displays flexibility and willingness to adjust work schedule to meet the needs of the participants and the agency.

PREFFERED EXPERIENCE, TRAINING, LICENSURE AND/OR CERTIFICATION:

·  Bachelor’s degree in social services or related field.

·  Excellent verbal and written communication and presentation skills.

·  Experience in Medicaid, Medicare and Social Security benefits.

·  Ability to build and sustain relationships with community stakeholders.

·  Understanding of, experience with and/or passion for individuals with disabilities and their families.

·  Ability to work effectively with multiple constituencies.

·  Critical skills for this position include organization and time management, problem solving and flexibility.

·  High initiative and ability to work independently or as part of many different teams.

·  Valid NC driver’s license.

·  Experience in Microsoft Office products (word, outlook and excel).

This position requires sedentary work exerting up to 10 pounds, light work exerting up to 20 pounds, medium work exerting up to 50 pounds. This position requires visual acuity to determine the accuracy neatness and thoroughness of the work assigned.
