Supporting Information for Revalidation arising between appraisal and revalidation dates in the first year of revalidation (to April 2014)

As revalidation requirements have only recently become generally understood, some supporting information will, particularly in year 1, not be available until after that doctor's most recent appraisal. The most common of these will be Colleague and/or Patient feedback :~

(Please use GMC for reference

Where the doctor has recognised what they need in time, or been advised by their appraiser, they should collect all necessary information to submit to their appraiser before their final appraisal before their revalidation date. The appraiser can then validate this at the appraisal meeting. Every effort should be made to achieve this, however we recognise that this will not always be possible.

In this situation your Responsible Officer (RO) will need to be informed that the feedback has occurred and that you have reflected on the content. You must discuss this with your appraiser, or if they are not able or willing to do this, a Supporting Medical Colleague, who should be an educationalist, trainer or appraiser, or a doctor who has appropriate training to give you feedback on your information.

You and your Colleague should then complete the attached template and forward to the BGSW area team so that the RO and their team can consider this. The latest date that this information can be received is two weeks before your allocated revalidation date. If you cannot provide validated information which meets GMC guidance before this time, your revalidation may be deferred. You should therefore make every effort to ensure that your patient and colleague feedback data collection is started well before this date.

Please submit the form to Samantha Green, GP Appraisal Administrator at
