THERE IS NO CUSHION OF TIME TO SUBMIT FORMS LATER. Please be considerate of this request.
DO WHATEVER YOU CAN TO ENSURE THAT YOUR PLAYERS GET THEIR FORMS SIGNED.

Thank you for agreeing to be the Department Coordinator for this year's Soccer Tournament. You will play a critical role in assuring that all employees who are interested in playing in the tournament have the opportunity to register and play.
TOURNAMENT PACKET
I've enclosed the following materials that you will need in registering employees and their spouses/partners:
A.Tournament flyer with Tournament information and registration form was sent by Countywide e-mail during the first week of June.
B.Roster sheet.
Please register every participant on the sheet(s) and complete the information.
C.Summary sheet.
This will summarize your team's information. If there are a number of coordinators for your department or team, please consolidate all information onto one sheet.
D.Registration form.
These should be completed by all participants and returned with the participant list and summary sheet.
E.A copy of the proposed Soccer Tournament Rules. These will be reviewed and finalized at the Team Captain’s meeting on 6/28.
F.Memo regarding Team Captains / Coordinator meeting on June 28th in Room 402, County Office Building in Redwood City.

JERSEY NUMBERS
Team must provide their own jerseys and numbers. Each team will select a color that all team members will wear while playing. The Team Captain may select a color only after the team roster and team fee are submitted to the Tournament Coordinator.

REGISTRATION FEE COLLECTION:
The team registration fee is $225.00. Participant fees cover all tournament costs. Participants should write checks or get money orders to pay for their registration fees. Check made payable to HEALTH & FITNESS. No cash, please.
