Austin High School Y.E.S. Contract

To participate in the Y.E.S. (Youth Expanding Services), students need to sign

and submit this contract.

The AHS Y.E.S Program offers opportunities for its students to get involved with voluntary service at a local community level; which is open to all and benefits the community in general.

The contract form and subsequent hour forms are to be submitted in the RED Y.E.S Box at the front office. These forms are collected twice a month and date stamped, recorded to the student file. Each file is confidential and can only be viewed by the student/guardian.

It is the responsibility of the student to correctly complete all YES forms, to submit them in a timely fashion, and to consult the sponsor if he or she has any questions. Forms submitted beyond 6 months from the date of service will not be accepted.

The Volunteer hours are recorded using the Y.E.S Hour form. The form must be signed by:

·  The representative of the organization/agency verifying the hours worked there.

·  The student.

·  The student parent/guardian.

The Y.E.S Program is voluntary and it’s an honor for students to be recognized in their final year of high school for their completion of the 100 Y.E.S hours. They are rewarded at the Senior Award Ceremony by getting a chord/pin/medal and a certificate. Many colleges now require community services as a criteria for acceptance.

All volunteer hours need to be from a non-profit agency and students may not be paid for any services rendered.

PLEASE REVIEW ALL GUIDELINES POSTED ON THE WEBSITE FOR ALL RULES AND REGULATIONS.

All service hours are subject to the Y.E.S. Sponsor approval.

I understand that to fulfill the requirements for Community Service, I must complete the required hours before the deadline for completion of my Senior Year. I further understand that I must submit documentation to the Y.E.S. program office and that if I should I have need for a copy of this information I must submit the request by email to the Y.E.S. committee.

If at any time during this process I falsify documentation, I or my guardians are rude or intimidating to any Y.E.S coordinator in person or via email. I understand that I will automatically be removed from the Y.E.S. program.
