Role purpose

The role holder will be expected to deliver effective, evidence-based community healthy lifestyle advice and support. This will be provided to those wishing to stop smoking, lose weight or increase physical activity in order to improve their health and wellbeing, meet the Government targets and support the reduction in health inequalities.

The Healthy Lifestyle Delivery Team are based around twogeographical localities and an early years setting, and will provide trained advisors across Cornwall and the Isles of Scilly in a variety of venues

Context

The aim of the Health Promotion Service is to improve the health of the population of Cornwall, in particular reducing health inequalities. This includes working with people from areas of high deprivation or those who are socially excluded. The service provides support and advice for others delivering work to improve health and wellbeing, i.e. health and social care staff, but also a range of projects and programmes working directly with local communities. The work focuses on achieving the 66 Public Health Outcomes by addressing issues such as promoting healthy weight, physical activity, smoking, mental health and wellbeing and sexual health
The aim of the Adult Healthy Lifestyle teamis to work with adults, either individually or in groups, to change lifestyle behaviours & improve health.This will include
Provide trained specialists in smoking cessation and healthy weight
  • Provide training for health professionals and others on evidence based advice and motivational support.
  • Provide a specialist service tomidwives to work with pregnant women and their families to decrease smoking and unhealthy in pregnancy

Accountabilities

  • Work with the colleagues throughout the Healthy Lifestyle team to achieve local smoking cessation targets
  • Work within the agreed smoking cessation protocols and guidelines to provide a professional and effective service
  • Provide effective, evidence-based smoking cessation advice to clients in one-to-one and group situations to help reduce smoking prevalence
  • To deliver Healthy Weight programmes to children, young people and adults with a particular specialism in Physical Activity / Healthy Eating / Weight Management.
  • Physical activity specialists will be required to deliver appropriate activity sessions within the programme of work. Healthy Eating Specialists will be required to deliver evidence based nutritional sessions, including practical cooking sessions.
  • Develop innovative and effective ways to engage individuals and groups to achieve and maintain a healthy weight through increased physical activity, healthy eating and behaviour change
  • Identify and provide information for individuals and groups relating to further activities available in the local area as exit routes from the delivery of Healthy Weight and smoking cessation Programmes.
  • To collect and collate the required information for individuals worked with eg demographics, weight and waist measures, physical activity levels and eating behaviours to support monitoring and evaluation data and to gather for follow up evaluation over a 2year period from the start of the intervention.
  • Maintain high quality standards at all times in relation to record keeping, appointment systems etc. to maintain protocol and quidelines
  • Work with the Health Promotion Service team, undertaking any other duties commensurate with the position to support the service
  • Develop an understanding of the issues relevant to adult health and wellbeing, including attending training courses and meetings to keep up to date with developments and changes
  • To research information on good practice on a local and national basis and disseminate to members of the team.
  • Attend health, obesity and smoking related meetings and conferences when required.
  • To work with a range of partners within Cornwall Council as well as other partner organisations and community groups, providing information on activities, referral pathways and best practice guidance.
  • To develop interactive resources promoting healthy weight which are inviting and engaging to a wide range of audiences.
  • Work closely with other members of the Health Promotion Service to ensure an integrated approach and consistent methodology is in place.
  • Complete all relevant paperwork and submit as required to the service to ensure it is in a timely and professional manner
  • Observe all rules and procedures in respect of fire, security and health and safety to ensure safety of postholder, service users and colleagues
  • To work flexibly within the role profile and carry out other duties as reasonably requested by the line manager.
  • This post requires a combination of sitting, standing and walking. There will also be a significant amount of manual handling when setting up equipment for session delivery.
  • This post will involve a significant amount of physical effort when delivering sessions, especially as part of the practical delivery of the physical activity sessions.
  • This post will involve weekly evening delivery with frequent driving
