While faculty members are expected to contribute in all three areas, it is not expected that every faculty member is active in every subcategory listed below. If there is nothing to enter in a particular subcategory, leave it blank.
I.TEACHING
(A)List courses taught in each semester, including information about the contact hours, enrollment, and nature of each course (e.g. required, service course, elective).
(B)Briefly outline teaching contributions tothe Liberal Arts Core and other university programs (e.g. honors, MLAS).
(C)Briefly outline any efforts to enhance student learning, including course redesign, new course development, and any professional development activities related to teaching improvement (e.g., participation in activities or consultation at the Center for Teaching and Learning, involvement in Inquiry ARC training, conferences on pedagogy).
(D) Briefly describe any publicly -engaged teaching activities, such as service learning.

(B)Listscholarly and creative activities for this year indicating the status for each (e.g. in progress, submitted for peer review). Please include work on grants (e.g., submitted/ accepted, funded/not funded), copyrights, patents, software created, editorship and peer review activities, attendance at professional meetings, professional development activities, professional consultancy, and research for new course preparation.
(C)List undergraduate research or MLAS theses presented or published under your supervision.

III.SERVICE
(A)List department service such as departmental committees or projects, assessment, or service as a liaison.
(B)List university service such as faculty governance, committees or task forces, ILS assessment, and
co-curricular activities to enhance faculty-student relationships (e.g. student club advisor, intramural coach, orientation).
(C)List community service that directly relates to your professional training and expertise (e.g. commissions/boards of directors, public lectures/workshops, work with local media, and any other publicly-engaged service).
(D)List professional service (e.g., leadership in a professional organization, moderation of conference panels, hosting of a conference).
