Yes. After this initial appointment, they will be considered CHC patients and can choose any CHC location as their medical home. / If they are current Washington Apple Health enrollees and it is a well child appointment, there should be no cost.
If they have already had a well child visit for that time period, there may be a cost for an office visit.
New Washington Apple Health enrollees may be billed for that day’s services, then reimbursed by the health plan. / Yes, but we will try to get the families (or at least the children) enrolled in Washington Apple Health at the time of the appointment.
If not insured, CHC patients can be on billed on a sliding fee scale for services received.

But, we are not a walk-in to the general public. / As long as their Washington Apple Health coverage is active, all preventative procedures, including recommended immunizations, are free of charge. / New patients need to be registered fully and to see an ICHS provider before getting immunizations.
Patients without insurance can apply for the ICHS sliding fee if they wish. Fees/discount levels are determined by a federally mandated sliding fee scale, based on family size and income level.
Patients should bring proof of income so we can determine their level of discount.

If they are unable to do so, we can assist them at the time of the appointment. / There is never an out-of-pocket cost for immunizations to patients who have active Washington Apple Health coverage. / Yes. Patients can receive immunizations and be billed according to a sliding fee scale. We will register them to document the visit, but they do not have to become our patients.

We have standing orders for our nurse to do immunizations. They do need to register as a patient because we need to have a chart on them to document the vaccines. / No. If they have active Washington Apple Health coverage, they receive state-supplied vaccine free of charge. / Yes. They would need to register as our patient to create a chart for the visit.

They do not need to become our patients. / There is no charge for immunizations given as part of a well child checkup for patients who have Washington Apple Health coverage. The child needs to be a SeaMar Monroe patient to schedule a well child checkup. / No. They don’t need to become our patients to qualify for the sliding fee scale, but they do need to bring proof of their income on the day of the visit.
