C. Field Trips
1. Field Trip Request – Scott – Football Camp – Georgetown College
Scott High School requests permission for 60 students to attend a Football Camp at Georgetown College in Georgetown, Kentucky on August 1-5. 2010. Supervision will be provided by Scott staff/coaches at a ratio of 1:7. Students and coaches will be staying overnight in the Georgetown College dorms. All meals are provided on campus at the Georgetown College Café. The approved Parent Permission Slip has been documented and sent to each student’s parent/guardian. There is no cost to the students. Transportation will be by district bus.
Recommendation: It is recommended that the Board approve the trip request.
D. Fundraiser
Fund-Raising Activity Request – J. A. Caywood Elementary
Caywood Elementary requests permission to conduct a school-wide fund-raiser from August 26-September 8, 2009. Funds raised will be used for student attendance incentives, technology needs, and fieldtrips - student support. Learner outcomes impacted are Goal 2 and Goal 3. The students will be selling candy bars (no door to door sales) from QSP, Inc. Box 2003 Ridgefield, Connecticut. Caywood’s Site-based Council approved the fund-raiser on May 26, 2010.
Recommendation: It is recommended that the Board approve this fundraiser request.
E. Bids
1. Bus Fluids Bid: Enc. 7
Following the June 21, 2010, Board Meeting the Bus Fluids Bid Award has been disputed. There was an error in interpretation of the Bid for Antifreeze and upon further examination the bid tabulation has been revised.
Recommendation: It is recommended that the Kenton County Board of Education resend the bid award for Bus Fluids from June 21, 2010 and award the bid as indicated on Enclosure.
V. DISCUSSION AND ACTION
1. Change Order for Purchase Order – Master Halco –
Scott High School Phase I – Site Improvements – BG 09-225 Enc. 8
This change order is to cancel P.O. #10001546-00 FY 2010 for the Site Improvements at Scott High School associated with Master Halco in the amount of $23,885.00. This P.O. will be cancelled because Master Halco will not provide materials in conformance with the specifications. The General Contractor, The Quandel Group, will obtain materials from another vendor and subcontractor. All costs (material, labor, sales tax, etc. will be paid for by The Quandel Group. The $23,885.00 will be added to The Quandel Group contract through a Change Order.
Recommendation: It is recommended that the Kenton County Board of Education approve the Change Order to cancel Purchase Order #10001546-00 FY 2010 with Master Halco for $23,885.00 contingent on Kentucky Department of Education approval.
2. Change Order No. 18 – The Quandel Group –
Scott High School – Phase I – BG 09-225 Enc. 9
This change order is the eighteenth of the construction of Site Improvements at Scott High School – Phase I associated with The Quandel Group. The original contract with The Quandel Group was $1,787,585.00. This change order for a deduct of $7,000.00 with previous change orders totaling $102,032.04 brings the contract total to date to $1,882,617.04.
Item No. 1 – Owner’s Request – To delete signage through General Contractor. The owner will work directly with vendor. DEDUCT: $7,000.00
Recommendation: It is recommended that the Kenton County Board of Education approve Change Order No. 18 with The Quandel Group associated with Scott High School Phase I – BG 09-225 for a deduct of $7,000.00, contingent on Kentucky Department of Education approval.
3. Change Order No. 19 – The Quandel Group –
Scott High School – Phase I – BG 09-225 Enc. 10
This change order is the nineteenth of the construction of Site Improvements at Scott High School – Phase I associated with The Quandel Group. The original contract with The Quandel Group was $1,787,585.00. This change order of $4,716.91 with previous change orders totaling $95,032.04 brings the contract total to date to $1,887,333.95.
Item No. 1 – Civil Engineer’s Request – Remove abandoned storm lines and backfill with suitable material. ADD: $2,701.54
Item No. 2 – General Contractor’s Request – Additional geotechnical observation time for installation of piers for the home bleachers. ADD: $2,015.37
TOTAL: $4,716.91
Recommendation: It is recommended that the Kenton County Board of Education approve Change Order No. 19 with The Quandel Group associated with Scott High School Phase I – BG 09-225 at a cost of $4,716.91, contingent on Kentucky Department of Education approval.
4. Change Order No. 20 – The Quandel Group –
Scott High School – Phase I – BG 09-225 Enc. 11
This change order is the twentieth of the construction of Site Improvements at Scott High School – Phase I associated with The Quandel Group. The original contract with The Quandel Group was $1,787,585.00. This change order of $23,885.00 with previous change orders totaling $99,748.95 brings the contract total to date to $1,911,218.95.
Item No. 1 – General Contractor’s Request – Provide the fencing, post, gates, concrete, and all necessary materials and labor and any additional expenses and taxes associated as required to install the fencing in accordance with Specification Section 323113. ADD: $23,885.00
The reason for this Change Order is due to the materials’ vendor (Master Halco) and the installation sub-contractor unwillingness to provide the materials listed in the specifications. This will result in no cost increase to the owner. The P.O. to Master Halco for $23,885.00 has been cancelled and the responsibility for providing and installing the fencing materials is being transferred to The Quandel Group.
Recommendation: It is recommended that the Kenton County Board of Education approve Change Order No. 20 with The Quandel Group associated with Scott High School Phase I – BG 09-225 at a cost of $23,885.00, contingent on Kentucky Department of Education approval.
