Copyright Transfer

Upon your acceptance, this agreement will constitute a transfer from you to the National Association for Healthcare Quality (NAHQ) of full ownership rights, titles, and interests, including the copyright and all rights subsumed thereunder in and to the work tentatively titled:

in all forms of media, whether known or hereafter developed, and in all languages effective when the work was accepted for publication.

Authorship

Your acceptance of this agreement will further signify that you represent and warrant that you are the sole author(s) and sole proprietor(s) of all rights in and to any portion of the aforementioned work; that the work is original and not in the public domain; that it has not been previously published or is being considered for publication elsewhere; that is does not violate or infringe on any personal or property rights of others, whether common law or statutory; that it contains nothing libelous or contrary to law; and that you have full power to enter into this agreement.

Your acceptance of this agreement will also constitute your agreement that the work contains no material from other copyrighted or unpublished works that have been used without the written consent of the copyright owner and/or of the owner of any other rights to or in such other works, and that you have obtained any such written consent as may be required and have delivered completed reprint permission requests to NAHQ. Any reproduced tables or figures will be accompanied by a note at the bottom of the reprinted table (or in the figure caption) giving credit to the original author and the copyright holder.

Your Rights

In exchange for the transfer of ownership of copyright, NAHQ will provide editorial services in preparing the work for publication. Notwithstanding the assignment of copyright to NAHQ, you:

  1. Will receive a complimentary copy of the issue in which your article appears; or a pdf file of the article if it is published on the Web exclusively.
  2. Retain patent rights, if any.
  3. May make photocopies of all or part of the work for use in classroom teaching.
  4. May make copies of all or part of this work for distribution in your place of employment.
  5. May use your original figures/tables/illustration/photographs from the work in future works of your own.
  6. May include the work as part of your dissertation, for noncommercial distribution only.

You agree that all copies and inclusions made under the above conditions will carry a notice of copyright and citation to the journal article (Example: ©2001 National Association for Healthcare Quality.
