Job Title: Licensed Practical Nurse

Summary of Job:Employee performs nursing functions in order to provide residents and patients with medical care.
Reporting Relationship: Employee may reportto the Director of Health Services, Resident Care Director and Registered Nurse.
Other Key Relationships:Administrator, Memory Care Coordinator, interdisciplinary team and the appropriate Chain of Command.
Essential Job Functions: Through employee’s own efforts:
- Uphold the PCSC Mission, Vision, and Values Statement in all aspects of work, leading by example and holding self accountable for exemplifying compassionate Christian service.
- Assign jobs, responsible for providing direction and exercise independent judgment.
- Implement and follow resident and patient plan of care.
- Perform rounds at beginning of shift and frequently during the shift to observe proper care and condition of the residents and patients. Communicate any issues which need addressing.
- Conduct weekly audits on any residents with bed sores or wounds, charting this information on the appropriate forms and inform Director of Health Services of findings and progress.
- Perform resident care duties as requested and required.
- Supervise, direct and assign duties to other staff as appropriate.
- Work closely and communicate with residents, patients and families in providing optimal individualized care; maintain availability to meet with families for consultation to discuss concerns, present and explain plans of resident care (including discharge), and to give moral support.
- Notify and communicate to family any change or pertinent information on residents and patients.
- Administer medications and treatments as ordered by the physician.
- Order medications in a timely fashion and keep medicine card clean and orderly.
- Review charts and MARs and address any issues or discrepancies.
- Communicate with on-coming shift any pertinent information on the residents and patients.
- Perform rounds and take-off orders correctly and accurately.
- Work closely with the physicians and discuss any medical issues related to residents and patients to ensure the services meet the needs of the residents and patients.
- Maintain a comfortable, clean and safe environment at all times.
- Coordinate all wellness therapy and other activities with other departments to promote socialization and overall wellbeing of the residents and patients.
- Perform venapunctures, I.V.s and other labs as ordered by a physician.
- Provide emergency care in conjunction with physician orders.
- Assist in evacuation of residents, if needed.
- Demonstrate and maintain competency in nursing skills.
- Oversee admissions, discharges and transfers of residents and patients.
- Initiate and maintain infection control policies and procedures.
- Employees are required to work weekends, holidays, and as per resident needs. This includes working other shifts, in all units, and/or staying past your shift until adequate staffing is met for resident need and regulatory compliance.
- Participate in all aspects of the Community including but not limited to: disaster preparedness teams, educational training, in-service meetings, annual training, and other trainings as assigned.
- Perform other duties as assigned.

Education: Licensed Practical Nurse in the State of South Carolina
CPR Certification

General Physical Requirements:
- Heavy Work: Employee is required to exert up to lOO pounds of force occasionally, and/or up to 5O pounds frequently, and/or up to 2O pounds of force constantly to move objects.
