Overview of Purpose of Role

The purpose of this role is to effectively manage services delivered to individuals in their own homes. To lead a team who support individuals in all aspects of their daily lives and in accordance with their evolving plans to enable them to live a good life.

-Conducts 1:1’s
-Manages all aspects of performance of direct reports
-Maintenance of personnel files & TMS

-Coordinating the delivery of service to the individual

-Deputises when & where required

-Delivery of service in line with evolving care and personal plans
-Compliance with clinical policies & advice & received training
Community Integration
-Ensures individuals are supported to establish, maintain and access links in the community through on-going discovery work
-Works closely with the individual and frontline care support team to continuously reflect on and improve the development of fitting roles.
-Acts as an advocate where appropriate.
