The eFormsprogram allows for an ACORD application to be created by entering data directly on an ACORDform and saving the data file (calledan eForm File)on a local system.
There are different versions of eForms available. You can learn more about the difference by review the on-line documentation once you are in eForms. See Appendix at the end of this document for a chart that outlines the functionality differences between the various versions of eForms, all EbixASP users will have access to the version with the heading of EbixASP at no additional charge.

UsingEbix eForms with EbixASP
GeneratingeForms from EbixASP
From the Policy Manager screen, select Forms > Application(eForm) as shown below.
The following screen opens, with options to Generate/Preview any one, combination of the listed ACORD forms, such as ACORD 125, ACORD 126 and ACORD 186,as currently implemented on EbixASP. Check the box(es) of the form(s) to export into the eForm Viewer andselect the ‘Generate E-Form’or ‘Preview E-Form’ button.
Selecting the ‘Generate E-Form’ button will create an entry to the Transaction Log with a copy of the ACORD form. A follow-up will be created similar to the forms generation process in EbixASP. Using ‘Preview E-Form’ will not create an entry in the Transaction Log, nor will it create a follow-up.
Note: If the eForm Viewer is not already installed on the PC, it can be downloaded directly from the link Download ACORD eForm View Application, which is provided at the bottom of the above screen.
Upon selecting the Generate or Preview E-Form button the following screen appears:
E-Mailing or Faxing eForms Application
To create an entry in the transaction log in EbixASP, select the Actionmenu for emailing or faxing a form.
- Post to EbixASP * Only the full version of eForms will have this option activated.
Allows the .EFRM file data to be posted into their EbixASP database. - Send E-Mail PDF (EbixASP)
The Transaction Log for the Client/Policy associated with the eForm application will be updated when the email is sent, with a copy of the PDF.
- Send E-Mail eForm (EbixASP)
The Transaction Log for the Client/Policy associated with the eForm application will be updated when the email is sent, with a copy of the eForm file.(The recipient will need the eForm Viewer)
- Send Fax PDF (EbixASP) *
The Transaction Log for the Client/Policy associated with the eForm application will be updated when thefax is sent, with a copy of the PDF. (*Only available with faxing option turned on in EbixASP)
The system will pre-select the policy number that is on the eForm. You may change the search criteria and/or search value to select a different Client/Policy once this panel appears:
When the client and/or policy is selected, click on OK and acommunications panel will appear where you can select the recipient(s) of the form:
Converting eForm to PDF
Select Tools > Export to PDF
The PDF displays and the file may be saved locally by selecting the save button.
Posting eForms to EbixASP
Only the Trial & Full version of eForms will have this option activated.
Please note for commercial lines policies the ACORD 125 applicant information section must be attached to any other ACORD form in order to post into EbixASP. Basic information required by EbixASP on the client and policy is gathered from the ACORD 125. If there is any other information required in EbixASP that may not be included on the eForm, prompts will display during the post to EbixASP process and the missing information must be provided.
Special Note: Commercial Package lines of business are indicated on the ACORD 125 in the “Indicate Sections Attached” area (top right). In order for the different coverage details to post into EbixASP, you must check the box for each LOB section you want to be part of the Package policy when it is posted into EbixASP. If the box is not checked, the details for that section will not be imported into EbixASP.

When starting with a new Application, the following fields are required to post a form without error

Select the option of how the policy should be imported into EbixASP.
New Business Quote (NBQ) imports into EbixASP under the ‘Quote’ tab on the Client Manager and will be in a pending status,which will allow you to continue to make edits to the policy details until you complete the Confirm Bound Policy process.
New Business (NBS) imports into EbixASP under the ‘Policy’ tab of the Client Manager and will be a completed process.
Renewal In Process (RIP) imports into EbixASP under the ‘Policy’ tab of the Client Managerand will be in a Renew Policy process, which will allow you to continue to make edits to the policy details until you complete the renewal process.
Renewal (RWL) imports into EbixASP under the ‘Policy’ tab of the Client Manager and will be a completed process.
Rewrite (REW) imports into EbixASP under the ‘Policy’ tab of the Client Manager and will be in a completed Rewritten status.
If there were any exceptions found during the import process, an Exception screen displays, such as shown below.
After processing all exceptions, the message (seen in red below) appears. This is an indication that the import into EbixASP has been completed.Once that message displays, manually close the window using the red X in the top right corner.
E-Mailing eForms (No Transaction log will be created in EbixASP)
To Email from eForms when not logged into EbixASP, select Tools > Send E-Mail eForm (the recipient will need the eForm Viewer) or Send E-Mail PDF. This will open from your local Outlook email program.

Appendix
eForms process reports can be found in Maintenance under the Download menu option. The eForms import process is very similar to how download is processed; some of the menu options have been combined:
- If any problem occurs during the import process, you can check the Transmission Log. It can be found under Maintenance, Download, E-Forms Trans Report
- If you exit out of the ‘Exceptions’ area of the eForm import process (such as selecting a client code or department) you can access the screen again from Maintenance, Download, Exceptions.
- To verify that the import was successful, you can check under Maintenance, Download, Successful downloads.
ACORD Forms currently supported by eForms for integration with EbixASP

One Key can be registered for one PC Only
You cannot use a Key on any other PC once it has been utilized, even if eForms is uninstalled from the PC. You will have to get the key unlocked by Ebix.
PC must attach to the Internet occasionally for eForms to verify registration and to receive updates. If eForms is unable to verify registration over a period of three days, it will work only in restricted mode (Reader Version options only)
Reader version is for a person who needs to fill-in the form and save it. If you send someone a blank form, they can complete the form and send back after completing it.
