Disposal of Plant - WHS Checklist

This checklist is to be used when disposing of plant by sale, trade-in, transfer, donation, decommissioning, dismantling, salvage or dumping. A copy must be provided to the purchaser/receiver of the plant. A copy must be provided to the purchaser/receiver of the plant and a copy retained by the Faculty/Portfolio concerned.

Note: The disposal method will determine which of the hazards below are applicable eg. some may not be present if the plant is to be dismantled, salvaged or dumped.

Are guarding/emergency stops/ safety devices in working order and adequate for the type of plant and the work it is used for?
Can someone be injured from electric shock due to damaged electrical leads/cables or damaged switches or emergency stop devises?
Can someone be injured by explosion of gases, vapours, liquids, dusts or other substances triggered by the operation of the plant or materials handled by the plant?
Can someone using the plant slip, trip or fall due to uneven or slippery work surfaces or lack of guard rails or other suitable edge protection?
Can someone come into contact with objects at high temperatures or be injured by fire when using this plant?
Is there a risk of crushing from parts falling off, or uncontrolled or unexpected movement of the plant?
Can someone be struck, cut, stabbed or punctured by this plant?
Are hazardous (or other) wastes/pollutants likely to be released by this plant?

Where plant is to be dumped, has it been made inoperable? eg. power cord removed
Is the plant being disposed of as required by appropriate waste management / environmental requirements?
Has the plant been removed from the register or a notation made that it has been disposed?
Documentation
The following documentation must be provided to the purchaser/receiver of the plant (except where plant is to be dumped directly by the University)
·  instruction manuals, information about safe use
·  maintenance records
·  details of any changes or modifications to the plant, including wiring diagrams
·  Condition of Acceptance of Plant form
A copy of the Condition of Acceptance of Plant form, signed by the purchaser/receiver of the plant, must be retained by the Faculty/Portfolio.
