Please ask the Insured to include the following on each check:

·  the Named Insured as stated on the policy, and

·  Federal Employer Identification Number (FEIN), and

·  Policy Number(s).

This will assure correct application of payments to the proper policies.

PRODUCER’S REMITTANCE OF PREMIUMS

If you remit premiums to the Company, remit the full premiums without withholding Commission – i.e., “Gross”. Please advise your sub-producers, if applicable. We will send you a check for your Commission. We will consider payments received “net of Commission” as “short payments”, and will cancel the policy for non-payment of premium.

Remit the premiums under any of the above three “Direct Billing” methods to the addresses shown.

PREMIUM FINANCE AGREEMENTS AND REMITTANCE OF PREMIUM

If the premium is paid by a third party, please ask them to:

·  remit payment under any of the above three “Direct Billing” methods to the addresses shown; and

·  send us a copy of the financing agreement as follows:
