6. BRIEF RESUME OF THE INTENDED WORK.

6.1NEED FOR THE STUDY:

As locomotion plays very important role in survival of an individual, the number of individuals suffering from pain at ankle and heel has increased rapidly. Ankle and heel are exposed continuously to strain and stress, as they are major weight bearing joints, leading to various kinds of injuries to different anatomical structure present in that region. Abnormal life style in so called modernization in urban area, where heel is continuously exposed to pressure and stress, due to obesity and by wearing wrong or high healed footwear’s etc, leading to above mentioned symptoms, and same is in the case of people living in rural area, as they walk bare foot in fields or on uneven surface leading to pain and inflammation of soft tissue of the heel.

Acharya sushruta has advised siravedhana at a distance of two angula above kshipra marma 1, 2, in vatakanthaka in which pain in heal is the major symptom, but anatomical significance of same is yet to be evaluated and established.

In present era study related to kshipra marma with respect to siravedhan around that area seems significant, for a good interpretation and better understanding of marma with respect to Rachana shareer and parinam, hence, there is a need for study of anatomy related to particular area between big toe and second toe.5

Siravedhana done on kshipra marma leads to marmabhighata3or death and the same when done above two angula of kshipra marma is treatment for vatakanthaka, vatashonith, etc, 4. The anatomical basis for this is to be clarified elaborately. Hence as per our classics, the siravedhana is advised at two angula above kshipra marma but which sira has to be considered for vyadhanakarma is still a matter of research. The anatomical changes that take place in vatakantaka, after siravedhana above kshipra marma is on the verge of discussion.

All these concepts are in need to be refined and applied practically to appreciate the course and relations of various structures that are participating in the process. Thus this study is an attempt to establish relevance between marma and the corresponding diseases. Hence in this study the complete evaluation of kshipra marma above two angula with respect to siravedhana will be done and the concepts are clarified after considering some observations. Hence the study has been undertaken.

6.2 REVIEW OF LITERATURE:

1.  As per SUSHRUTHA SHAREERA sthan, chapter 6th, verses 26, explained about the measurement and location of kshipra marma.

2.  In SUSHRUTHA SHAREERA sthan, chapter 8th, shloka no.16/a, acharya sushutha has explained the siravedhna at the site of kshipra marma in various diseases.

3.  As per SUSHRUTHA NIDAN sthan, chapter 1st, shloka 76, he explained about nidan of vatakanthaka.

4.  In MADHAVA NIDAN, vatavyadhi nidan, shloka no.61, he explained about the nidan of vatakantaka.

5.  As per SUSHRUTA sharer sthan 6th chapter, verses 33, he explained about the type of death due kshipra marma injury.

6.  In ASHTANGA SANGRAHA, sutra sthan, 36/8, explained about siravedhana above kshipra marma in various diseases.

7.  ASHTANGA HRUDAYA, nidan sthan, vatavyadhi nidan, shloka 53, explained about the nidan of vatakanthaka.

6.3 Objectives of the STUDY:

1.  To dissect the cadaveric foot for structural understanding of kshipra marma above two angula.

2.  To evaluate the anatomical relevance of siravedhana two angula above kshipra marma in vatakanthaka.

7. MATERIALS AND METHODS:

7.1 SOURCES OF DATA:

1.  Cadaveric foot dissection will be done, to know the underlying structures of two angula above kshipra marma.

2.  Diagnosed patients of vatakanthaka will be selected from OPD and IPD of shalya tantra and panchakarma department of Shri sidharoodha charitable hospital attached with NKJAMC Bidar. .

7.2METHODS OF COLLECTION OF DATA:

METHODOLOGY:

·  It is an observational study, comprising of pre-test and post-test design. siravedhana will be done under the supervision of shalya tantra and panchakarma specialist.

·  For this study 30 patients will be taken up, the sign, symptoms and structural changes before and after treatment will observed /recorded.

POORVAKARMA / ·  Amapachana
·  Snehapana (Till samyagsnigda laxana)
PRADHANA KARMA / ·  Patient made to lie down in supine position.
·  Tourniquet will apply above the ankle joint.
·  Then the site of Siravyadha i.e. 2 angulies above the kshipra marma is cleaned and siravyadana will be done.
PASCHAT KARMA / ·  The place of Siravyadha is applied with haridra churna and vital data checked.
·  Advised to follow Pathya

PERIOD OF STUDY:

The duration of study will be taken for 30 days after treatment. The pattern of pain, (KANTAKAVATA PEEDA), walking time before and after treatment will be observed and recorded on 1st day, 2nd day, 15thday and 30th day after treatment.

INCLUSION CRITERIA:

1.  Patients with classical features of vatakantaka.

2.  Patients fit for siravedhana.

EXCLUSION CRITERIA:

1.  Bleeding disorders, calcaneal fracture, and diabetes.

2.  Classically unfit patients for siravedhana.

ASSESMENT CRITERIA:
