6. BRIEF RESUME OF THE STUDY
6.1 NEED FOR THE STUDY
Dysmenorrhea is a common gynecological problem faced by women during their reproductive period. It causes significant discomfort and anxiety there by affecting inter-personal relations. Primary Dysmenorrhea is a symptom complex with cyclical pelvic pain radiating to back and legs associated with gastrointestinal and neurological symptoms occurring before or during menstruation or both with out any pelvic pathology.
The incidence of primary dysmenorrhea become higher in competitive modern era, due to change in life style and faulty food habits. Systemic review of studies from developing countries revealed that about 25%-50%of adult women and about 75% of adolescents experience pain during menstruation. 5-20% reporting sever dysmenorrhea with increased intensity of pain enough to prevent them from perform their daily routine.
Udavartini yoni vyapath can be defined as pain full menstruation, mainly Vata type. The aggravated Apana Vata moving in reverse direction fills the yoni resulting in discharge of Artava with difficulty. And immediate relief of pain following the onset of the menstruation.¹
For the diseases of yoni, Apana Vata is consider as the primary cause and it is said that without the vitiation of vata, manifestation of yoni roga are not possible. In Ayurveda line of treatment is to correct Vata vitiation and Basti is line of treatment in Vata Roga, hence is the treatment of Udavartini Yoni vyapath¹.
Charaka has mentioned Yapana Basti which is indicated in rajo vikara particularly Mustadi Yapana Basti which is Shukra, Bala and Mamsa Janana and cures all the Rajo Vikara. Therefore this study is under taken to evaluate the role of Mustadi Yapana basti in the management of primary dysmenorrhea.
6.2 REVIEW OF LITERATURE:
DISEASE:
NIRUKTI: Ut (upward/reverse direction) +avarta (Turing round /circular movement)
Due to upward movement of the menstrual flow this condition is called as Udavartini.
Historical review: No reference of the word Udavartini Yoni vyapath can be traced either in pre Vedic or Vedic period. The term is found in Samhita period.
In Caraka Samhita chikista stana, Yoni vyapath chikista adhyaya gives elaborative description about nidana,samprapti,laksana and chikista¹.
In Sushrutha Samhitha uttara stana, Yoni vyapath pratiseda adhyaya explains briefly that along with difficulty there are pains of vata will be there.
Other Samhithakaras like Asthanga Sangraha³, Madava Nidana7,Yoga Ratnakara mentions8 the disease Udavartini Yoni Vyapath in detail.
DRUG/THERAPY
MUSTADI YAPANA BASTHI:6
Reference-charaka samhitha siddi stana-uttarabasti siddi adyaya
Ingreadients:mustha,usira,bala,aragvada,rasna,manjistha,katurohini,trayamana punarnava, vibitaki,guduci stira panchamula.-1pala each(48g each)
8 fruits of madanapala-yavakuta churna
Jala-1adaka (3094)
Ksheera 2 prastha (96ml)
Mamsa Rasa-1/4 of ksheera.(50ml)
Equal quantity of Madhu and Ghritha.
Satakusuma,maduka,kutaja,pala.rasaanjana,priyangu as kalka.
Palashruthi:
Shukramamsabalaprada,ksathaksheena,gulma,shula,visamajwara,udavartha.kushi shula,mutrakuriicch,raktha vikara,rajo vikara,and also this basti is sadyo bala prada.
Dose: 12 prasutha

6.3. OBJECTIVE OF STUDY:
1. To evaluate the effect of Mustadi Yapana Basti in Udavartini yoni vyapath(primary dysmenorrhoea)
MATERIALS AND METHOD:
30 patients will be selected randomly and given with mustadi yapana basti for 7 days after 7 days of menstruation for 3cycles.
SOURCE OF DATA:
A minimum of 30 patients diagnosed for having dysmenorrhoea attending OPD and
IPD of S.D.M college of Ayurveda hassan shall be selected for study.
METHOD OF COLLECTION OF DATA:
Patient who are fulfilling the diagnostic and inclusion criteria will be selected for study.
DIAGNOSTIC CRITERIA:
The diagnosis will be established on the basis of patient’s history, symptoms and USG of abdomen and pelvis.
Symptoms
Ruja/kasta raja srava-pain starting few hours before or just with the onset of menstruation and lasting upto 12-24hrs
Spasmodic pain in the lower abdomen and radiating to lower back and anterior medial of thighs.
U.S.G - To rule out underlying pelvic pathology.
INCLUSION CRITERIA:
- Female patients of the age group of 16-30 years
- patient with cyclic pain.
EXCLUSION CRITERIA:
1. Patients with pelvic pathology diagnosed by USG and clinical examination.
2. Patients with medical and surgical complication.
TREATMENT:
Mustadi Yapana Basti
Method of preparation of Basti:
75g of Kvata chroona will be taken in 1200ml of water and boil on mild fire to prepare 300ml of kvatha. To this 200ml of milk will be added to prepare and Ksheera paka will be prepared.
. 120ml of maksika will be taken in kalva yantra to this 6g of powdered saindava lavana will be mixed slowly with continuous triturating to this 120ml of Maha narayana thaila added continuous trituration will be done. When solution are properly mixed 20g of kalka dravya will be put in solution,trituration will be continued. Finally 200ml of prepared kvata and 100ml of mamsa rasa will be added to the solution and homogenous mixture is done by mantana proceude.
Dose: 540ml
Method of administration:
Yoga Basti will be given as follows-
A / N / A / N / A / N / A1 / 2 / 3 / 4 / 5 / 6 / 7
A=Anuvasana- Mahanarayana thaila.
N=Niruha- Mustadi Yapan Basti
Mode of administration:
Stanika Snehana and Swedana will be done over Gluteal region. For patient isleft lateral position with left knee flexed, prepared Basti solution is administered slowly.
ASSESMENT CRITERIA:
Patients will be assed on severity and duration of pain.
FOLLOW UP STUDY:
Patients will be followed 15 days once after the treatment.
