STUDENT, GUARDIAN, SPONSOR, AND PRINCIPAL RESPONSIBILITIES

Student Responsibilities –

Behave in a respectful, responsible and safe way at all times
Be aware of other members on the DMS team, and take care of each other
Learn actively and ask questions
Fundraise and make all payment deadlines
Arrive at meetings on time or let me know ahead of time if you’ll be late or absent

Guardian/Chaperone Responsibilities –

Behave in a respectful, responsible and safe way at all times
Fundraise and make all payment deadlines
Support your student and the DMS team; follow all school rules
If you are chaperoning the trip, you are fully responsible for your student only
Arrive at meetings on time or let me know ahead of time if you’ll be late or absent

DMS Staff Sponsor Responsibilities –

Behave in a respectful, responsible and safe way at all times
Be aware of other members on the DMS team, and take care of each other
Learn actively and ask questions
Arrive at meetings on time
Enroll participants, organize the Close Up itinerary and travel plans, and share Close Up information with the DMS team
Supervise students and serve as a liaison between students and guardians, while preparing for and attending the program

Principal Responsibilities –

Behave in a respectful, responsible and safe way at all times
Review the sponsors itinerary and travel plans
Provide opportunities for the DMS team to meet and fundraise on Delta School District property
Ensure that the field lesson does not interfere with students academics, attendance or behavior (includes meetings and fundraising)
Makes the final decision on student are eligibility to attend the field lesson
Students who are suspended or expelled from school during the 2012-2013 school year may lose the privilege of attending the field lesson
Support the student, guardian and sponsor relationships

STUDENT, GUARDIAN, DMS SPONSOR AND PRINCIPAL CONTRACT

All students, guardians, and the DMS sponsor, Hailey Hancock, promise to follow the Delta Middle School and Delta County School District rules outlined in the student planners while preparing for and participating in the field lesson. Students, guardians, and Mrs. Hancock are expected to meet payment deadlines, uphold their school responsibilities, and behave respectfully at all times while preparing for the field lesson and during the field lesson. If there are any issues, a meeting with the student, guardians and Mrs. Hancock will be scheduled with Delta Middle School’s principal, Mrs. Koceski.

While participating on the field lesson, if a student violates any school rules, Mrs. Hancock has permission to notify the guardians that the student is being sent home, on their own, at the expense of the guardians. Once a student has violated the terms of this contract, Delta Middle School and the sponsor, Hailey Hancock, are no longer legally responsible for the student.
