This is a self-assessment form; it is your responsibility to ensure you know how to use this piece of equipment prior using it in clinical practice. If you have any issues that you are not sure of please speak to your Practice Educators or contact the Resuscitation Training Department. You must complete this form prior to using this equipment. This defibrillator is only to be operated by Doctors and Nurses who have attended an ALS / ILS course guidelines 2005. Transthoracic pacing should only be carried out by staff trained in this skill.

1. Are you familiar with how to check this unit prior to use?
2. If the fault light is displayed can you rectify the problem?
3. Can you turn the unit on:
(a)To monitor
(b) To Defibrillation mode
(c) To Pacing mode.
4. Can you plug in the pads?
5. Are you familiar with Defibrillator safety:
  • Ensure that patients chest is dry before sticking on the pads. Clear any other liquid surrounding the patient.
  • Remove excess chest hair using a safety razor.
  • Drug Patches. Must be removed from the surface of the chest.
  • Apply pads to the patients bare chest and follow the voice and visual prompts. When instructed to stand clear remove free flowing Oxygen 1metre away from the patient.
  • Proximity of rescuers You must ensure that no one is near to the patient whilst the unit is charging and when you (are instructed) press the shock button. Remember to say ‘Stand Clear!’ loudly and check that no one is touching the patient

8. Do you know how to turn on/off the synchronisation mode?

Special Considerations

Pacemakers. Should not pose a problem but if they are located where the Anterior pads should be, either move the pad the width of a pad away from the pacemaker or place the pads in the anterior/posterior position.

Internal defibrillators: The patient should be defibrillated externally despite the presence of an internal defibrillator. Defibrillator function will need to be checked should the patient survive.
