The deposit for the tennis key is $25.00, and the pool bathroom key is $25.00. Both are refundable upon return. This key is to be used for your family and guest for the purpose of playing tennis. Pool bathroom key is optional. Bathroom key option selected ______

Please read and initial the following below:

_____ I am the legal adult resident of the property listed above, and will always be in attendance during use of the tennis courts and bathrooms.

_____The key is used to unlock the gate from both the outside and inside the gate.

_____ Shut and lock the gate when you are leaving for the day.

_____ The courts are used for tennis play only.

_____ The bathroom key is used to unlock the swim pool bathroom back door, near the basketball courts when the pool area is closed. When the pool is open, resident should only access from the door in the pool area, and the back door should not be used. (if applicable)

_____ Shut and lock the swim pool bathroom back door every time it is used. If a guest is using the bathrooms, resident must be the person to lock and unlock the doors, or accompany the resident into the pool area front door. (if applicable)

_____ As the resident above, you are responsible for any damage or abuse caused to the pool bathroom after use by resident or if left unlocked. (if applicable)

_____ The tennis signup sheets located on the DVCA office window should be filled out when reserving, or using the tennis courts at any time.

_____ Status of resident’s assessments must be current to use any of DVCA Recreation Center complex, including tennis courts & bathrooms.

_____ Please report any court damage or misuse of the area to the office manager.

_____ No alcohol is to be consumed on the tennis courts, and no Firearms and/or weapons are allowed on any of DVCA property.

NOTE: Violation of this agreement can result in the loss of use or suspension of the tennis courts and Recreation Center complex.
