By signing this form, I, the parent/guardian of the above-named player registering to play for Dedham Girls Softball League (DGSL), hereby give my approval to participate in any and all DGSL activities including transportation to and from the activities.
I know that participation in softball may result in serious injuries and protective equipment does not prevent all injuries to players. I hereby waive, release, absolve, indemnify and agree to hold harmless DGSL, Amateur Softball Association (ASA), the organizers, sponsors, supervisors, participants and persons transporting my child to and from activities for any claims arising out f any injury to my child whether the result of negligence or any other cause.
I agree to return upon request the uniform and other equipment issued to my child in as good condition as when received except for normal wear and tear. I agree to pay any replacement fee assessed by DGSL for not returning the uniform by the requested deadline. I have read the Zero Tolerance Policy provided with this registration and by signing this form I agree to adhere to this policy.
I will furnish a certified birth certificate, if requested, for the above-named player to DGSL officials.

ZERO TOLERANCE POLICY
On rare occasions, some parents, spectators and coaches exhibit poor behavior. At this time, we want to make the following very clear: we are taking a “Zero Tolerance” approach to such behavior displayed by parents, players, spectators or coaches.
- Nobody (not a parent, player,spectator, Coach, nor Assistant Coach) may approach an Umpire to argue a call or play made. In any sport, there may sometimes be bad calls and you have an obligation to act in a GOOD SPORTSMANLIKE manner, whether you agree with the call or not. We do not feel it is the place of any parent, player, spectator or Coach to challenge calls or make confrontational, inappropriate or disruptive remarks to Umpires. We ask you all to remember that we value our Umpires and that some of them are of high school age and only a few years older than your own daughters. Remember, in a few years that could be your daughter behind the plate calling balls and strikes.
- You are not to argue, swear, scream or loudly disagree with any other parent, player or Coach. Our children should never witness such displays.
- No parent, player, spectator or Coach is expected to say anything unless it is a positive comment.
If any of the above occurs, the Umpire will stop the game and identify the violators to the Coaches for the purposes of removing parents, player, spectator or Coach from the field. Once removed, play will resume. The out of order parent, player, spectator or Coach will risk causing his or her own team to forfeit the game. We have instructed our Directors, Coaches and Umpires to contact the Police Department to assist in any such dismissal.
We sincerely hope to never have to remove even one parent, player, spectator or Coach from a game, but it is our obligation to maintain control, safety and a healthy atmosphere for the entire team. Let us all work to improve conduct by eliminating these unfortunate incidents and unbecoming behaviors.
