The undersigned ………..…………… in his/her charge as legal representative of ……………. ……………… as documentation annexed to the “Otto per Mille” project entitled……………………… declares to be:
1. Aware that incorrect or false declarations/certifications/annexes to the present application, if discovered after the approval of the project by the Tavola Valdese, can invoke the annulment of the Otto per Mille financing. This in accordance with the judicial processes of the Tavola Valdese, in its capacity as moral, non profit, religious, charitable and educational body as an administrators of the funds assigned by the Waldesian Church.
2. Aware that the non-observance of the policy and current laws in the country where the project is being carrying out, either fully or partially financed by the Otto per Mille funds, will invoke the immediate suspension of the financing.
3. That you have read the rules for access to the Otto per Mille Funds of the Waldensian Evangelical Church (Union of Waldensian and Methodist churches), to accept their content, and that the project for which funding is requested falls within required by provisions for access to Otto per Mille’s funds.
4. to be aware that any incomplete request or form not accompanied by the provided documentation will not be consider.
5. That any contribution given for the purposes of the withholding of 4% in art. 28 of DPR 209/09/1973, N 600 (tick the appropriate box):
(1) Is subject to withholding of 4% as an advance IRES;
(2) Is not subject to withholding of 4% as an advance IRES, since the institution does not work and the contribution of business itself is not destinated in support of commercial activities;
(3) Is not subject to withholding of 4% as an advance IRES, as for the purchase of auxiliary capitals.
The undersigned further commits to the following:
a) To allow inspections of the activities and audits on behalf of personnel entrusted by the Tavola Valdese. Inspections can be held without forewarning.
b) To return to the Tavola Valdese, sums improperly received and the amounts not used in case of abandonment or suspension of the project, with the addition of accrued interests from the date of the transfer of the financing.
c) To promptly inform the Tavola Valdese, with a written communication, about any changes or modifications in the project, as well as in the data, financial or other conditions as originally quoted in the application.
d) To promptly send a final detailed report that also includes the final settlement of accounts.
