On behalf of the Decatur County Youth Soccer Club, I would like to invite you to support our local program. The Decatur County Youth Soccer Club is a non-profit youth soccer club, offering a spring and fall program for children 4-16. We strive to provide a wholesome environment where children can learn and grow in the sport of soccer. We would not be able to accomplish this without the support of our community sponsors.

At this time we are seeking to fill our Spring 2016 and Fall 2016 team sponsorships. We offer many levels of participation. If you would like to support a team, your donation of $200 provides your company logo on the team shirt for one season. For $350 your donation provides your company logo on the team shirt for two seasons.

We also have banners available for $150 for one year, with a $75 annual renewal donation, or $200 for two years.

Additional funds are always needed to maintain the fields, pay referees, provide scholarships and upkeep of equipment. If you would prefer to make a monetary donation that would be greatly appreciated as well. Whatever your budget, there is an opportunity to help support a great local organization.

2016 Spring/2016 Fall DCYSC Sponsorship / Donation Form

Decatur County Youth Soccer Club will display the name of participating sponsors on the

team(‘s) jerseys according to the following schedule:

One Season Sponsorship (Either Spring or Fall) $200

Two Season Sponsorship (Spring and Fall) $350

Sponsorships are available for U-6, U-8, U-10, U-12, and U-14/16 teams. Sponsors are permitted to sponsor more than one team. All players keep their shirts.

Sponsorships will be assigned on a first come, first served basis. To sponsor the team of your choice, sign up early.

•The DCYSC complex is located at 601 Ireland Road, Greensburg, Indiana.

•The one-year time period will begin on the date of first scheduled game of the season. Banners will be taken down during the winter months. Renewal payments must be received before the first scheduled game of the following season.

•Sponsor is responsible to provide adequate artwork of logo in a digital format. If artwork or logos needs to be digitized, the price will increase depending on the complexity of the design.
