  1. How far away will the new place be from the existing Pathways location?

The new building is just across the parking lot from the current site. Your Personal Services Coordinator (PSC) can help you plan your transportation if you need that. You would use the same route that you are currently using.

  1. Who will help me with transportation to the new office?

Your Personal Service Coordinator will be able to assist you in planning your transportation and making sure that your specific needs are met so that you can access services without interruption.

  1. Will I lose my Personal Services Coordinator (PSC) or Psychiatrist with this change?

No changes in service team members will be made based on this move. You should still have the same Personal Service Coordinator and Psychiatrist. The program structure will also remain the same. The only change will be the location at which some of the services will be provided.

  1. Will there be an interruption in my services during the move?

No, it is our goal to make the move as quickly and efficiently as possible without any interruption in services to you.

  1. I like the old building, why do we need to move?

The current site was a temporary space as we waited for the completion of our new and permanent site. We are excited to say the new location is cleaner and more up to date than the last location. Restrooms and therapy rooms will be more conveniently located. There will also be more group rooms allowing for simultaneous group sessions. There is also parking located in a beautiful park-like setting. Most exciting is the addition of an on-site pharmacy in the new lobby. This will provide a very easy and convenient way to get your prescriptions filled and refilled.

  1. Who can I talk with about any concerns or questions regarding this move?

The first person to contact is your Personal Services Coordinator who can answer many of your questions or connect you to the appropriate staff member for questions they are unable to answer. You may also obtain additional information or direct your questions to (916) 283-8259.

  1. What can I do to make sure I get what I need during this change?

You can help by making sure you ask for what you need from our staff. Be sure and share yourconcerns and questions as soon as possible, and work with your PSC to have your questions answered. Your PSC will ask you for a current mailing address so that we can update our records and ensure that we can properly contact you via USPS mail service.

It is our goal to make the move to the new site as seamless and easy for you as possible. We are happy to be able to provide you with a great new space to participate in services and are eager to have your feedback.

We would like to thank you for your continued patience and dedication throughout this process. Should you have any questions or concerns, please do not hesitate to contact us.

Sincerely,

Al Rowlett, LCSW, MBA, CPRP

Chief Executive Officer
