Dear Parent/Guardian,

Health is an important factor in the academic success of all students. When students enter ninth grade, they continue to experience periods of growth, physical changes and social maturation requiring proper nutrition, adequate sleep, and regular medical and dental care.

·  SCHOOL PHYSICAL EXAMINATION – Students will be participating in our physical education program, which stresses physical fitness and includes strenuous activities. For ninth graders and continuing high school students, we recommend regular physical exams by a licensed medical practitioner. School Physical Examination form may be downloaded from www.whs.tusd.org.

·  ATHLETIC SPORTS PHYSICAL – A physical exam is required of all students planning to participate in the athletic program. School Physical Examination form completed after June 1 will meet the CIF requirement for participation in athletics for the upcoming school year. Physicals are good for one calendar year only.

·  RECOMMENDED IMMUNIZATIONS – Consult your health care provider about the following immunizations:

o  Meningococcal – Meningococcal blood infections and meningitis are potentially fatal and are easily spread by kissing, sharing drinks, coughing, or sneezing. One shot is recommended at age 16 or over. Consult your Health Care Provider.

Hepatitis A Series – Due to a high incidence of Hepatitis A in California, it is now a recommended vaccination for students. Consult your Health Care Provider.

Tdap (Tetanus/Diptheria/Pertussis) – This booster is a new requirement effective July 1, 2011 for all 7th through 12th grade students.

o  Varicella – A second dose is recommended if you have never had chickenpox.

·  HEALTH HISTORY FORM – The Health History Information form is enclosed. It must be completed annually and returned during the registration process. Please fill this form out completely including medications on the reverse side so that we can prepare to meet your student’s health needs at school.

·  Medication while at school -- State law requires written permission from both parent and physician before medication can be taken at school. If medication is required during school hours, an Authorization for Medication at School will need to be completed and the medication will need to be stored in the Health Office. Students who require emergency medication (Inhaler or Epi-pen) may carry their treatments with them; however, a Contract to Carry Life-Sustaining Medications on Campus must also be completed. For Epi-pens, physician must complete the Physician’s Authorization for Management of Anaphylaxis and Epinephrine Autoinjector Administration at School. All forms may be obtained from the Health Office or downloaded from www.whs.tusd.org.
