Dear Ooltewah High School Parents

Believe it or not, it will soon be time to start collecting athletic physicals for next school year. Physicals for the 2016-2017 athletics seasons can begin to be accepted as long as they are dated after April 15th. Having said that, the way that your child’s physical will be turned in by you and then stored by Ooltewah high school is going to change beginning April 15th. We are excited to introduce a new and easier process for completing the health history forms required for sport participation, called e-PPE (electronic Pre-Participation Evaluation). e-PPE replaces the paper-based forms with an electronic Pre-Participation Evaluation solution for submitting and managing health information. We at Ooltewah High School have partnered with a company called Privit profile. Privit Profile™ is compliant with international privacy laws, including HIPAA and COPPA, ensuring your child’s information is secure.This new process will help us increase the health and safety of student athletes and enhance the security and accessibility of the health information when needed.

The Major differences that you will notice is that all forms, except for the one your doctor signs and a consent form, are all on filled out online and an e-signature can be applied using your smart phone or computer. This means that you will not need to re-fill health information forms every year. You will just need to sign the necessary forms and update information at the beginning of every year. If you have multiple children at Ooltewah high school you will not need to make separate accounts for each child, you will just need to add them as a family member to your account. Once your child has gotten the doctors signature to play sports you can upload the form using a scanner or the camera on your phone or tablet. Once this is done your child’s information will be viewable to only the coaches of the teams that they are members of.

It is our hope and intention, by moving to this technology, to have your child’s information stored in a secure location, and eliminate the need for duplicate copies of physicals and other forms kept by multiple coaches. This will be the only way that Ooltewah high school will accept and store physicals moving forward.
