Dear CCSD Support Staff employee,

We are pleased to announce the open enrollment period for your 2017 benefits is from November 4, 2016 to December 2, 2016.

The enclosed benefit information is provided to help you make informed decisions about your medical, dental and vision plans. It includes an Open Enrollment Meeting Calendar, Dental Flyer, and Medical Benefit Overview.

Important things to know:

  • The easy-to-use Medical Plan Welcome Guide includes important information on plan designs and how to access care.
  • Please review the enclosedfive health insurance plan optionsin the overview for 2017, including HMO 1, HMO 2, POS 1, POS 2 and a PPO plan as well as deduction amounts based upon 20 payrolls.
  • Payroll deduction amounts for these plans are effective January 1, 2017.
  • Employee Only insurance deductions for all insurance plans will not change for 2017.
  • HMO II insurance deductions will not change for 2017.
  • New: Dependents now have coverage until the age of 27 for medical, dental, and vision.
  • New: Dependents enrolled in the HMO plans now have access to covered services outside of the HPN service area if the dependent is enrolled in an accredited university, college, or vocational school in the United States. Proof of full-time student status must be provided at time of enrollment to HPN. Please contact HPN Member Services at (702) 240-8707 for details.
  • Preventive health care services will continue to be covered at no cost.
  • Dental and vision coverage will be included with the CCSD medical insurance plan for employees and eligible dependents who are enrolled in medical insurance.
  • Beginning January 1, 2017, UnitedHealthcare Dental will be your new dental insurance carrier. Dental benefits will remain the same; however, you will receive a new dental ID card in the mail. Please review the enclosed UHC dental flyer for additional information, and remember to let your dentist know you have a new dental plan in 2017.
  • MetLife is now the Life and Long-Term Disability carrier for Support Staff employees.
  • NowClinic 24/7 telemedicine service isincluded in all plans.
  • New custom website for CCSD Support staff available atMyHPNCCSD.com.
  • Access to The Life Connection (TLC) Employee Assistance Program
  • All changes made during this open enrollment period are effective January 1, 2017.

Open enrollment change forms will be available in the Employee Self-Service (ESS) system from November 4, 2016 to December 2, 2016.

/ Open enrollment change forms must be submitted to the Clark County School District Benefits Department no later than December 2, 2016.

If you do not have any open enrollment changes, NO ACTION IS REQUIRED. Your current medical, dental, vision, life, AD&D and long-term disability plan elections will remain the same.
