Meeting Minutes
Date: June 9, 2016
Location: Cantalician Center – TriMain Building
- Cantalician Center Employment Services Presentation and Tour
- Darren Lisicki (Director of Employment Services) and Melisa Granville (Coordinator of Community Prevocational Services) presented on the services offered:
- Community Prevocational Services
- 60-70 individuals/day
- Biweekly paychecks for individuals
- Integrated with refugee population
- Transportation Assistance Available – Independence encouraged
- There are current openings – contact Melisa – 710-5469 (Fax: 710-5499, email )
- Send: ISP, IEP (if applicable), Psychological, Universal App, Physical, Front Door/Service Authorization if you have it (it’s faster if you have it already, but not required).
- Encouraged to set up a tour first to see if it’s a good fit.
- Supported Employment
- Have some locations where staff is on site working with individuals (transition from CPV to independent community employment)

- Chairs from these committees are meeting 6/13/16 to see if there’s anything that can be done to improve processes
- CAS Roll Out
- CAS staff have been visiting agencies to explain the process. Agencies are getting calls now that individuals have been selected. All have been individuals in IRAs so far.
- PCP Regulations
- We are asking that everyone bring their assessment tools to the August meeting to share
- We discussed how people are meeting with the individual the month before the ISP meeting to talk about PCP and do assessments. Some are doing it via phone. For children, people are mostly doing this with parents. Some have chosen not to focus on children at this point. Some are doing formal assessments and others are not.
- We discussed if anyone is doing sign offs for hab providers for the ISPs and only Heritage and Cantalician are doing this at this point. Neither are chasing after hab providers to sign, just showing that they sent it to be signed.
- We discussed where people are documenting that the individual chose where and when the ISP meeting is – all but one agency is doing this in the case notes. The other agency puts it in the ISP.
- Agency updates:
- DQI Audits are continuing across the region. Best of luck to all!
- FSS Letters are going to be sent separately from the Front Door since it is not a waiver service in order to stop holding up those services while waiting for waiver approval.

- Transitioning students
- Some are still waiting for service authorization from the Front Door
- There is confusion among all regarding whether MSCs should be reaching out to agencies regarding openings and sending information about individuals before service authorization is received. It has been hard for agencies to receive information a week or two before graduation and be able provide services right away. Additionally, students are missing out on possibilities of “shadowing” or “trying out” services. It was suggested that this committee invite DDRO Transition staff to a meeting in the fall to discuss how we can all best serve the transitioning students for 2017.
