Guidance notes for data collection plan form

Questions / What to consider
Why is the data being collected? / What is the data being collected for? Is it meaningful? I.e. will it help you evidence an improvement?
Who will collect data? / To ensure consistency, data should be collected by as few people as possible. Where possible one individual should have the responsibility for data collection.
What data will they collect? / A clear operational definition is required for any measures and the measurement plan can help you with this. If a percentage is used, what are the numerator and denominator?
Where will they collect the data? / From a computer system, from patients’ notes? Is the data from particular wards or the whole Trust?
When will they collect the data? / How frequently will the data be collected? Daily, weekly, monthly etc? Is it collected at a certain time e.g. midnight bed state. If using point prevalence data, will it be collected at the same time of day, on the same day of the week etc?
How will the data be recorded? / A simple and clear data collection tool, agreed before you collect your baseline data, will ensure only the essential information is collected and that the same data is collected every time.
Is the data validated (if reported elsewhere)? / If data is reported in multiple forums it is important that the same data is reported everywhere.
Who is the data gatekeeper? / Who is responsible for the data/ the report that generates the data? Who can answer any queries regarding the data?
Will we count every event or take a sample? / Is the data being collected for all patients or a sample of patients? If a sample, how is the sample chosen?
How will data be presented? / Will the data be presented on an SPC chart? (recommended for improvement projects)

How will the data be recorded? / Copy of report saved each month, data recorded in excel
Is the data validated(if reported externally)? / Not applicable
Who is the data gatekeeper? / HIVE report written by QI data analyst
Will we count every event or take a sample? / Every discharge counted /
How will data be presented? / SPC chart produced by QI analyst
