Customer Login Screen

This is a first page (screen) to be seen by the customer when he/ she choose to set an appointment on QAMC web page. The screen is set up in such a way that it prompts the customer with fields that gather the customer name and password. The screen is also able to direct new users into the appropriate pages. Nevertheless, existing users who somehow forgot their passwords can also be directed into necessary screen.

CUSTOMER PASSWORD RETRIEVAL SCREEN

This page is specifically for existing customers who forgot their password. The page assists the customer in retrieving his/ her password. The customer verifies his/ her information which includes his/ her name and address (Street, City, State, Zip).

CUSTOMER VERIFICATION SCREEN

After being accepted by the previous processes, the customer verifies his/ her information which includes his/ her address (Street, City, State, Zip). Here, the customer can also update his/ her information incase they appear to be incorrect. After making necessary changes (if any), the customer specify the vehicle and the service desired. When finished, the customer submits this information by using the schedule request button which automatically takes his to the next screen.

This screen is for scheduling appointment as far as the times and days are concerned. The customer is prompted with various choices of days and times during those days that are desirable to the customer and convenient to the management.

DETAIL CONFIMATION SCREEN

This screen enables customers to confirm what they chose in the previous screens. It displays all chosen information by the customer including his/ her vehicle, chosen date and time, chosen service etc.

CONFIRMATION AND SURVEY SCREEN

This screen confirms the customer appointment using a unique conformation number provided by the system. Here the customer also has a choice of rating the QAMC online application system according to their online experience. This information will be united with customer feedback which is usually produced two days after the appointment.
