Curricular Practical Training Application Form

Instructions:

Practical Training (PT) is a vehicle devised by the US Immigration and

Naturalization Service (INS) for foreign students (F1 visa) to gain work

experience as part of their academic training.

Curricular Practical Training (CPT) must be recommended by the

student's advisor, and is subject to approval by the CS Practical

Training Committee, the Department Chairman, and the International Student

Affairs office.

CPT is intended for internships in the student's field of study before

graduation. For full-time employment, there is a total limit of 12

months (including any time on either CPT or OPT); for part-time employment,

there is no total time limit.

A student on CPT is generally approved for part-time employment during

the academic year (full-time employment only in rare cases) or for

full-time employment during breaks (including summer break). However,

students on CPT during the academic year must remain FULL-TIME

students during that period.

Before filling out the form below the student should read the

departmental guidelines at

and obtain the Curricular Practical Training Recommendation form and

the Immigration and Naturalization Service form I-538 from the

International Student Affairs office.

The form below should be filled out by the student and her/his advisor

and submitted to the graduate secretary preferably one semester in advance.

The application will be routed to the Chair of the Computer Science

Practical Training Committee and/or the Department Chair for evaluation

and approval.

It should be accompanied by the Curricular Practical Training Recommendation

form from the International Student Affairs office, which should also be

filled out by the student and his/her advisor, and the Immigration and

Naturalization Service form I-538 filled out by the student. The dates and

information on all forms should correspond.

Note that curricular practical training is normally only approved for at

most a semester. Employment should be only on a part-time basis during

the Fall and Spring semesters and students should remain registered

for a full academic load during the period. During the summer semester

full-time curricular practical training may be appropriate.
