We are excited that our data entry system officially went live this week! The official site address is: Our training website will no longer be available at this time.

Next Steps: If you are a Facility Administrator (Project Lead or Project Coordinator), look for an email from Jul 27 with the subject: ‘Welcome to the Cardiovascular Surgical Translational Study (CSTS)’ in your inbox. Please use the account and password to log onto the data entry system and create units and create accounts for your Unit Data Entry personnel (UDE) for them to begin data entry. Please also enter the number of HSOPS survey respondents to begin the HSOPS process. Support materials for the data entry system can be found on our website at
